Preferences on Vital and Nonvital Tooth Bleaching: A Survey Among Dentists from a City of Southern Brazil

This study evaluated the preferences of general dentists regarding vital and nonvital tooth bleaching therapies and investigated whether the time of clinical practice and postgraduate training influence these options. A cross-sectional study was conducted using a questionnaire with closed questions applied to dentists (n=276) of a mid-sized city of the southern Brazil (Pelotas, RS). Information was collected regarding sociodemographic variables, level of specialization and time since graduation. In addition, options regarding bleaching therapies including the first choice of material, technique and clinical practice for vital and nonvital tooth bleaching therapies were included. Data were submitted to descriptive analysis and the associations were evaluated using chi-square and Fisher exact tests (a=0.05). The response rate was 68% (n=187). At-home bleaching therapy was broadly preferred (78.1%) over in-office (21.9%) bleaching. For at-home bleaching, most dentists answered to use 10% carbamide peroxide (CP) (40.2%) and >30% hydrogen peroxide (HP) (31.7%) for nonvital therapies. The majority of dentists with post-graduation training preferred at-home bleaching techniques (p=0.003). At-home bleaching therapy was also more indicated by younger dentists. No association was found between the choice for nonvital bleaching therapies and time since graduation (p=0.532) or continuous education (p=0.083). In conclusion, at-home bleaching was preferred over in-office therapies; 10% CP and >30% HP were chosen as first option agents to treat discolored vital and nonvital teeth, respectively. The time in clinical practice and the level of specialization affected dentists' choices only for vital tooth bleaching treatment.

ABSTRAKTujuan penelitian ini untuk mengetahui perbandingan kekerasan mikro dentin mahkota setelah aplikasi berbagai bahan bleaching intrakoronal. Jenis penelitian ini merupakan penelitian eksperimental laboratoris. Sebanyak 32 gigi premolar pertama mandibula tanpa karies, telah diekstraksi, dipotong 2 mm di bawah cemento-enamel junction dibagi dalam 4 kelompok dan bahan bleaching dimasukkan ke dalam kamar pulpa, yaitu kelompok A -45% karbamid peroksida, kelompok B -35% hidrogen peroksida, kelompok C -sodium perborat dikombinasikan dengan aquadest, dan kelompok D -aquadest. Akses kavitas ditutup kemudian disimpan di dalam aquadest dengan suhu 37 o C. Prosedur bleaching dilakukan pada hari ke-0, 7, 14 dan 21. Setelah 28 hari, mahkota gigi dipotong secara longitudinal dan salah satu bagian ditanam di akrilik. Nilai kekerasan mikro dentin mahkota diuji menggunakan Vickers microhardnes tester. One way ANOVA dan uji LSD digunakan untuk mengevaluasi pengaruh berbagai bahan bleaching intrakoronal terhadap kekerasan mikro dentin. Hasil penelitian menunjukkan nilai kekerasan mikro dentin mahkota pada kelompok A sebesar 45,04 VHN, kelompok B sebesar 45,42 VHN, kelompok C sebesar 55,22 VHN dan kelompok D sebesar 55,63 VHN. Kesimpulan dari penelitian ini terdapat perbedaan kekerasan mikro dentin mahkota yang signi kan antara kelompok 45% karbamid peroksida dan 35% hidrogen peroksida dengan sodium perborat dikombinasikan dengan aquadest, sedangkan antara kelompok 45% karbamid peroksida dengan 35% hidrogen peroksida tidak terdapat perbedaan yang signi kan.Kata kunci: bahan bleaching intrakoronal, dentin mahkota, kekerasan mikro ABSTRACT: Comparing microhardness of dentine crown after application of various intracoronal bleaching agents. The bleaching efficacy of common bleaching agents and deionized water treated with non-thermal atmospheric pressure plasma in the pulp chamber for nonvital tooth bleaching was evaluated. A total of 120 extracted human maxillary first incisors were stained using human blood. Teeth were randomly divided into eight groups (n = 15). In the first four groups, teeth were bleached using 35% hydrogen peroxide gel, 37% carbamide peroxide gel, 2:1 (w/v) sodium perborate paste, and deionized water for 30 min. In the remaining groups, bleaching agents were treated with non-thermal atmospheric plasma for 5 min inside the pulp chamber. Overall color changes (∆E) were determined using Commission Internationale de L'Eclairage Lab Colour System. The plasma-assisted tooth bleaching has not increased tooth temperature beyond 37°C. Bleaching efficacies of bleaching agents were significantly improved when treated with non-thermal atmospheric plasma compared to their application (P < 0.05). A remarkable bleaching effect was obtained when bleaching agents were substituted with water and when treated with non-thermal atmospheric plasma. Non-thermal atmospheric plasma treatment could be a novel tool for activation of bleaching agents in the pulp chamber for nonvital tooth bleaching procedure. Moreover, water could be used as a novel bleaching agent when treated with the non-thermal atmospheric plasma to eliminate possible risks which might arise from peroxide-containing agents.
