“…Since Indonesia - as a developing country - has a considerable amount of low economic population with poor oral hygiene maintenance, this can lead to a serious and possibly fatal complication over the result of conventional palatoplasty procedure such as infection, wound dehiscence, risk of fistula, scarring, hematoma and partial necrosis ( 5 , 9 , 10 ). Due to this fact, one of the main consideration when planning surgical procedures using BFP as pedicled graft in our cases is the excellent healing of BFP with minimal complication.…”

“…Despite the absence of comparative studies, a global case series involving 223 patients showed the capacity of the BFP technique to prevent palatal fistula and cover the raw bone surface after sealing the palatal flap. It showed good results also as salvage option for secondary palatal defects with a comprehensive failure rate of 0.9% with no velopharyngeal insufficiency claims, no impairment in the function and growth of the palate observed, and no aesthetic deformity reported [35, 38-50]. A contraindication to BFP use exists in cases of anterior palatal defects, although Pinto and Debnath [51] have described a surgical technique using BFP for lining a nasal defect during a releasing pushback palatoplasty.…”

“…Buccal fat may also be used. 16 These flaps certainly avoid free-flap procedures and provide medium amount of tissues. In case of larger defects, bilateral flaps from both the cheeks can be harvested and some authors state, donor site morbidity was virtually absent, when buccal fat pad is used for donor site closure, which is not our experience.…”
