Effectiveness of automated and live phone reminders after mailed-FIT outreach in a pilot randomized trial

Abstract: The effectiveness of annual mailed fecal immunochemical testing (FIT) outreach is highest when return rates are optimized, which is aided by patient reminders. In a pilot patient-randomized controlled trial in two western Washington clinics of the Sea Mar Community Health Centers, we compared the effectiveness of two phone-based approaches to mailed FIT outreach reminders. In fall 2016, patients ages 50–75, due for colorectal cancer screening, and with a visit in the previous year at either of two clinics, wer… Show more

“…While telephone-based interventions have been shown to promote healthy behaviors in previous research 26 , 27 and are frequently practiced in clinical settings, 28 , 29 we did not observe a difference in TCT enrollment between groups on the basis of receipt of an additional telephone call to discuss the FRP. However, we did find a strikingly high proportion of TCT enrollment in both study arms during the RCT, well above the accepted standard consent rate among patients with cancer.…”

“…This compares with 81% reach for live phone calls reported by our team in a similar safety net setting and 26%-48% reach in previous published reports. 6,18,[25][26][27] Further improvements in phone call reach might be achieved by increasing the number of call attempts or expanding the hours calls are delivered. The 81% reach we report for text messages was slightly higher than reported in previous studies, where reach ranged from 51% to 78%.…”

“…Phone calls may serve as a valuable prompt and can verify a patient's address and empanelment status in health systems, particularly in underserved communities; however, calls can be a resource-intensive intervention. [8][9][10] While advanced notification calls before FIT outreach are effective, 7,[11][12][13] they may not be necessary for all patients, especially for those more likely to complete a FIT. [14][15][16] Though patients most likely to complete a FIT are those with prior FIT completion, 17,18 studies investigating the effectiveness of pre-FIT advanced notifications did not focus on this subgroup.…”
