The aim of this study was to evaluate the skin problems and dermatological life quality of the health care workers ( HCWs) due to personal protection equipment (PPE) use, who are at high risk for Covid‐19 infection. A questionnaire about HCWs' PPE use, their skin symptoms, and prevention, management methods and Dermatology Life Quality Index (DLQI) was fulfilled. The median age of 440 participants was 33.5 (21.0‐65.0) years old. Skin problems were found to be 90.2%, the most common were dryness, itching, cracking, burning, flaking, peeling and lichenification. The presence of skin problems ( P < 0.001) was higher in those who did not use moisturizers. Of all, 22.3% (n = 98) stated that the use of PPE increased the severity of their previously diagnosed skin diseases and allergies ( P < 0.01). Only 28.0% (n = 123) stated that they know the skin symptoms that may develop by using PPE. The proper hand washing rate was higher as education level increased ( P < 0.001). Skin problems were higher in those using mask with metal nose bridge (p:0.02; p:0.003, respectively). As the mask using period prolonged, acne was more common (p:0.02). DLQI was significantly affected in women ( P = 0.003), and with increased skin problems related to PPE ( P < 0.001).It is important to organize trainings on prevention and management of possible skin symptoms due to PPE use according to guidelines. Occupational allergic contact dermatitis from systemic drugs

Background: Health-care workers (HCWs) and professionals working in the pharmaceutical industry are at risk of developing occupational allergic contact dermatitis (OACD) from systemic drugs (or drug intermediates).Objectives: To study demographic characteristics and identify systemic drugs responsible for OACD in patients investigated for contact allergy during the period 2001-2019.Methods: In the study period, 9780 patients were patch tested with the European baseline series, sometimes with additional series, and other relevant potential allergens. All patients with a positive patch-test reaction to systemic medication exposed to at work were included for further analysis.Results: Of 1248 HCWs examined in our clinic, 201 suffered from OACD. In 26 (13%) dermatitis was caused by skin contact with a systemic drug: 19 nurses, five chemists working in the pharmaceutical industry, one physician, and one veterinarian. In total, 45 positive patch-test reactions to 20 different systemic drugs were found, with tetrazepam (n = 11), ranitidine hydrochloride (n = 5), and zolpidem (n = 4) being the most frequent. Three pharmaceutical chemists were sensitized to a drug intermediate. The lesions were mostly localized on the hands, but often also on the face, as airborne dermatitis.

ZusammenfassungDer BGN als Unfallversicherungsträger für Beschäftigte im Nahrungsmittel- und Gastgewerbe werden aktuell jährlich etwa 2000 berufsbedingte Hauterkrankungen gemeldet. Beschäftigte in der Gastronomie sind dabei am häufigsten betroffen, gefolgt von Bäckern, Konditoren, Backwarenverkäufern, Beschäftigten im Bereich der Fleischwirtschaft und in der Lebensmittel- und Getränkeindustrie. Als Hauptbelastungsfaktoren für die Haut an Händen und Unterarmen gelten Feuchtarbeit sowie Kontakt zu Nahrungsmitteln. Kumulativ-subtoxische Kontaktdermatitis, atopisches Ekzem und deutlich seltener auch die allergische Kontaktdermatitis sind die häufigsten Primärdiagnosen im Hautarztbericht. Wird seitens der Bezirksverwaltung der BGN ein Heilbehandlungsauftrag erteilt, erfolgt zeitnah ein telefonisches Erstprofiling mit dem Betroffenen. Die Teilnahme an einem eintägigen Haut-Basisseminar wird obligatorisch angeboten. Bei schweren berufsbedingten Hauterkrankungen oder langwierigem, chronisch rezidivierendem Verlauf erfolgt je nach Region die Einbeziehung des beratenden Hautarztes der BGN, die Vorstellung in einem speziellen Hautschutzzentrum, die Teilnahme am regionalen Hautpräventionsprogramm der BGN bzw. eine individuelle Tertiärpräventionsmaßnahme (TIP).
