INTRODUÇÃOA mitomicina C (MMC), antimetabólito alquilante, atua ligando-se ao DNA, acarretando ligações anômalas e quebras em sua estrutura, inibindo a mitose, a síntese protéica e levando à morte celular (1) . Inicialmente, foi utilizada como terapia adjuvante após exérese de pterígio (2) . Chen em 1983 foi quem primeiro relatou o seu emprego em cirurgias antiglaucomatosas (3) . A partir de então, vários estudos evidenciaram que seu uso no per-operató-rio reduz a cicatrização da ferida cirúrgica (4)(5)(6) . Após a introdução da MMC na trabeculectomia (TREC), o sucesso no controle da pressão intra-ocular (Po) a longo prazo aumentou consideravelmente, com redução da falência da bolsa fistulante (7)(8) .A eficácia da MMC associada à cirurgia antiglaucomatosa depende de sua concentração, do tempo e da forma de exposição (9)(10)(11)(12) . O uso da MMC resulta, na maioria das vezes, na ocorrência de uma bolsa avascular (isquê-mica), difusa e grande, o que explica, em parte, sua efetividade. Porém, seu uso está associado a maior índice de complicações, como hipotonia (causada por hiperfiltração, descolamento de coróide ou vazamento de humor aquoso), atalamia, câmara anterior rasa, catarata, endoftalmite, phthisis bulbi, etc (7)(8)(13)(14)(15)(16) .Trabeculectomia com mitomicina C em pacientes com glaucoma congênito refratário Objetivo: Relatar os resultados do uso de mitomicina C (MMC) na cirurgia do glaucoma congênito refratário. Métodos: 44 olhos de 30 pacientes com glaucoma congênito primário refratário, submetidos a trabeculectomia com MMC entre 1993 e 2002 no Serviço de Glaucoma do Hospital São Geraldo, foram estudados retrospectivamente, por meio de análise dos prontuários com seguimento mínimo de seis meses. Resultados: A média de Po reduziu-se de 21,7± 7,2 mmHg no pré-operatório para 12,2±6,8 mmHg no tempo médio de acompanhamento pós-operatório de 42,8 meses (p<0,001). A complicação registrada foi a hipotonia crônica (Po<6 mmHg) em quatro olhos (9,1%), um deles com vazamento de humor aquoso pela bolsa fistulante (teste de Seidel positivo). Não se detectou relação estatisticamente significativa entre a incidência de complicações e o sexo (p=0,14), idade à época da cirurgia (p=0,65), Po pré-operatória (p=0,29), número de colírios no pré-operatório (p=0,39) e número de cirurgias prévias (p=0,94). Conclusão: O uso de MMC na cirurgia fistulante do glaucoma congênito primário mostrou-se eficaz na redução da Po, porém, a incidência de hipotonia ocular pós-operatória foi alta. To evaluate the long-term efficacy of intraocular pressure reduction and complications of Ahmed glaucoma valve (AGV) implantation in children with primary congenital glaucoma. Methods:The medical records of patients with primary congenital glaucoma who underwent AGV implantation with a minimum follow-up of 6 months were reviewed. The primary outcome measure was cumulative probability of success, defined as intraocular pressure greater than 5 mm Hg and less than 23 mm Hg and at least a 15% reduction from the preoperative intraocular pressure, without serious complications, additional glaucoma surgery, or loss of light perception.Results: Thirty eyes of 19 children with primary congenital glaucoma who underwent AGV implantation with a minimum follow-up of 6 months were reviewed. The children had a mean (SD) age of 1.8 (2.6) years, a mean (SD) preoperative intraocular pressure of 28.4 (6.7) mm Hg, and a mean (SD) follow-up time of 57.6 (48.0) months. The cumulative probability of success was 63% in 1 year and 33% in 5 years. After a second AGV implantation, the cumulative probability of success was 86% in 1 and 2 years and 69% in 5 years. Hispanic ethnicity (P=.02) and being female (P=.005) were associated with increased risk of failure.Conclusions: Thirty-three percent of AGV implantations in children with primary congenital glaucoma were successful after 5 years of follow-up. With the implantation of a second AGV, the 5-year success rate increased to 69%.
