Background: The ocular pulse amplitude (OPA) is the difference between the minimum and maximum values of the pulsatile intraocular pressure (IOP) wave contour. The OPA depends on ocular perfusion and IOP, which are both affected by a trabeculectomy (TE). The aim of this study was to investigate how the OPA changes after TE and whether an early change in OPA can be used as a prognostic marker for a successful long-term outcome. Methods: Fourteen consecutive patients (26-84 years old) with medically uncontrolled primary open-angle or pseudoexfoliation glaucoma were included in the study. IOP and OPA were measured with a dynamic contour tonometer before and after TE on days -1, +1, +7, +14, +21, +28, +42, +56, +70, and +84. The OPA of the contralateral eye was used to control for variations in systemic haemodynamics. TE was regarded as successful if a persistent drop in IOP of at least 20% without the use of IOPlowering treatment was achieved. Data were analysed using receiver operating characteristic curves, Kaplan-Meier survival curves and Mann-Whitney two-sample analysis.

ABSTRACT.Purpose: To evaluate the effects of dorzolamide/timolol fixed combination (D/T) compared to latanoprost on intraocular pressure (IOP) and pulsatile ocular blood flow (POBF) in primary open-angle glaucoma (POAG) patients. Methods: Thirty patients with POAG were randomized in an open-label, crossover study. Intraocular pressure reduction was achieved by 4 weeks medical therapy with D/T twice daily or latanoprost 0.005% dosed once in the evening. During a 4-week run-in and a 4-week wash-out period between study arms, patients ceased use of all other glaucoma medications and used timolol maleate 0.5% twice daily. Primary efficacy variables were IOP and POBF. Results: There was no difference in baseline IOP and POBF parameters between the two study arms. Both D/T and latanoprost statistically significantly reduced IOP by 4.6 mmHg (p < 0.0001) and 3.75 mmHg (p < 0.0001) and increased POBF by 2.048 ml/second (p = 0.0030) and 2.147 ml/second (p = 0.0009), respectively. Repeated measures ANOVA detected significant changes in POBF with treatment (p = 0.0361). Dorzolamide/timolol fixed combination statistically significantly increased pulse volume by 0.767 ml (p = 0.0087), while latanoprost therapy had no significant effect (p = 0.2407). Conclusions: Both drugs had similar effects in terms of IOP reduction. Dorzolamide/timolol significantly increased pulse volume while latanoprost had no effect. Further studies are necessary to establish whether the enhancement of choroidal blood flow can prevent glaucoma progression.

Purpose: The purpose of this study is to examine the changes of the retina and the intrinsic survival signals of the retina by brinzolamide (Azopt®) and travoprost (Travatan®) in a rat model of chronic ocular hypertension. Methods: Chronic ocular hypertension was induced by cauterization of three episcleral veins. Terminal deoxynucleotidyl transferase-mediated dUTP-nick end-labeling (TUNEL) staining was performed and the expression of glial fibrillary acidic protein (GFAP) was examined to evaluate changes of retinal ganglion cell (RGC) apoptosis and glial cell activation. Western blot analyses of the bcl-2 family and extracellular signal-regulated kinases (ERK) were done to identify changes of the intrinsic survival signaling pathway in the retina. Results: GFAP expression and TUNEL staining revealed significant decreases in RGC apoptosis and glial cell activation after brinzolamide and travoprost administration; bcl-2 and bcl-xL expression were significantly increased after intraocular pressure elevation and it was further increased with brinzolamide and travoprost treatment. This enhancement of survival signaling may have contributed to the decrease in RGC apoptosis. However, the role of ERK signaling was not significant. Conclusions: Decrease in retinal damage and increased intrinsic survival signals suggests the neuroprotective potential of brinzolamide and travoprost in an animal model of chronic ocular hypertension, but further studies are required.
