Background. Axillary lymph node (ALN) staging is the most effective method to evaluate the condition of patients with breast cancer, their choice of treatment options, and prognosis. The sentinel lymph node (SLN) status assessment is the key to sentinel lymph node biopsy (SLNB) in patients with breast cancer. The choice of tracer and tracer injection sites affects SLNB. Objective. This study mainly analyzes the best tracer for SLNB and the best choice of tracer injection site. Methods. A total of 165 breast cancer patients who underwent SLNB were selected and injected with methylene blue or 99mTc-labeled sodium phytate or nanocarbon 20 min before biopsy. The number of SLNs detected by different tracers in different injection sites such as peritumoral tissue (PT) and subareolar area (SA) was counted, and the sensitivity, specificity, and positive/negative prediction rates were recorded and compared. Results. The detection success rate, average detection number of SLNs, and detection accuracy of the nanocarbon tracer were higher than the other two. The detection sensitivity, specificity, and positive and negative prediction rates of nanocarbon for SLNB were also higher than those of the other two tracers. When comparing the performance of tracers in different injection sites, it was found that the detection of three tracers injected in the SA was better than the injection in the PT. Conclusion. For women with early-stage breast cancer, nanocarbon can be used as the preferred tracer for SLNB to determine the status of the patient’s ALNs, and the areola area can be used as the best injection site.

Objective: In this meta-analysis, we investigated the safety and efficacy of carbon nanoparticle (CNP) trace-guided lymph node (LN) dissection during radical gastrectomy. Methods: Literature on CNP tracing compared with non-CNP tracing in radical gastric cancer (GC) surgery was searched from PubMed, EMBASE (Ovid platform), Web of Science, and the Cochrane Library from the establishment of the library until October 2022. This meta-analysis was performed according to the preferred reporting items for systematic reviews and meta-analysis guidelines. Available data regarding the number of LNs dissected, number of metastatic LNs dissected, other surgical outcomes, and postoperative complications were analyzed in a pooled manner. Stata software (version 12.0) was used for the present meta-analysis. Results: This analysis included 7 studies with a total of 1827 GC patients (551 and 1276 in the CNP and non-CNP groups, respectively). The results of the meta-analysis showed that the CNP group had more intraoperative LNs detected [weighted mean difference (WMD) = 6.67, 95% confidence interval (CI): 3.71–9.62], more LN metastases (WMD = 1.60, 95% CI: 0.09-3.12), and less intraoperative bleeding (WMD = 11.33, 95% CI: 6.30-16.37) than the non-CNP group, all with statistically significant differences ( P < .05). For postoperative complications (odds ratio [OR] = 0.88, 95% CI: 0.52-1.48) and operative time (WMD = −11.60, 95% CI: −40.53-17.34), there was no statistically significant difference between the 2 groups ( P > 0.05). Conclusions: CNP was a significant tracer for the LNs of GC. It increased the number of LNs harvested while reducing intraoperative blood loss, without increasing the operative time or postoperative complications. CNP tracer-guided lymphadenectomy is considered safe and effective for gastrectomy.

Use of a Carbon Nanoparticle Tracer to Detect Sentinel Lymph Nodes for the Precise Treatment of Endometrial Carcinoma

We evaluated the usefulness of a carbon nanoparticle (CNP) tracer for the detection of sentinel lymph nodes (SLNs) for the precise treatment of endometrial carcinoma (EC). Among patients who presented at First Affiliated Hospital, College of Medicine, Zhejiang University, and Xiangshan First People’s Hospital Medical and Health Group between February 2019 and February 2022, we selected 90 patients who required surgical treatment for EC. During the operation, CNP was injected into the cervix by the two-point method, and black-stained LNs were identified as SLNs and resected. Then, statistical analyses were conducted to evaluate SLN occurrence area, CNP traceability, SLN detection rate, as well as diagnostic data such as false negative rate (FNR), sensitivity (SEN), specificity (SPE), positive/negative predictive value (PPV/NPV), and accuracy (ACC). CNP tracing detected SLN mainly at the external iliac (45.09%) and obturator (36.36%) regions. CNP tracing of SLNs was successful in 80 patients, with an SLN detection rate of 88.89%. Eight cases (10.00%) were positive for SLNs, with an FNR of 12.50%. The SEN, SPE, PPV, NPV, and ACC values for CNP detection of SLNs were 87.50%, 98.78%, 87.50%, 98.78%, and 97.78%, respectively. Our results indicate that CNP is an effective tracer for SLNs. When operating on EC patients, CNP tracing of SLNs can help to judge lymph node metastasis and regional distribution, providing a reliable basis for the determination of surgical scope.
