미만성 경화 변종 갑상선 유두암(diffuse sclerosing variant of papillary thyroid carcinoma, DSPTC)은 1953년 Crile과 Fisher[1]에 의해 처음 보고된 매우 드문 아형으로 전체 갑상선 유두암의 0.7~5.3%를 차지한다[2~4]. 병리학적 으로는 한쪽 또는 양쪽 엽의 미만성 침범, 광범위한 림프절 전이, 현저한 섬유화, 편평상피화생(squamous metaplasia), 다수의 사종체(psammoma body), 림프구 침윤 등을 특징으 로 한다[5,6]. 임상적으로 전형적인 갑상선 유두암보다 젊은 여성에서 호발하며, 진단 당시 국소 림프절 전이 및 원격 전이 발생률 이 높고, 치료 후 재발 빈도가 높다. 비록 드문 변종이지만 하시모토 갑상선염과 임상적으로 유사하므로 진단에 있어 매우 유의해야 한다는 점에서 중요성을 갖는다. 저자들은 내원 1달 전부터 생긴 전경부의 종괴로 내원한 접수일자: 2008년 7월 9일 통과일자: 2008년 10월 14일 책임저자: 박근용, 건양대학교 의과대학 내과학교실 환자와 직장 건강 검진에서 우연히 발견된 갑상선 병변을 주소로 내원한 환자에서 각각 DSPTC를 경험하였기에 문헌 고찰과 함께 보고하는 바이다. 증 례 1. 증례 1 환 자: 김OO, 24세 남자 주 소: 내원 1달 전부터 만져지는 전경부의 종괴 현병력: 환자는 평소 건강하게 지내던 중, 1달 전부터 전 경부에 만져지는 종괴가 발생하였고 빠르게 커지는 양상을 보여 개인병원을 방문하였으며 하시모토 갑상선염 의심 하 에 갑상선호르몬제(씬지로이드)를 복용하였으나, 점차 크기 가 증가되어 이에 대한 정확한 검사 및 치료를 위하여 본원 내분비내과 외래를 방문하였다. 과거력 및 가족력: 고혈압, 당뇨병, 간염 및 결핵 등은 없 었고 흡연 및 음주는 하지 않았다. 갑상선 질환의 과거력도 없었다. 가족력에서 갑상선 질환 및 다른 질환의 병력은 없 었다. ABSTRACTThe diffuse sclerosing variant of papillary thyroid carcinoma (DSPTC) is a rare histological subtype characterized by diffuse involvement of one or both thyroid lobes, widespread lymphatic permeation, prominent fibrosis, squamous metaplasia, abundant psammoma body and lymphatic infiltration. This subtype usually occurs in young female, and exhibits a higher frequency of cervical and distant metastasis. DSPTC clinically resembles Hashimoto's thyroiditis, and often delays the correct diagnosis. We experienced two patients with DSPTC: the one patient presented with a neck mass lasting for a month, and in the other patient, a thyroid lesion was incidentally found during a medical examination.
