ÖzetEndoftalmi katarakt ameliyatının en ciddi postoperatif komplikasyonudur ve tanı konduğunda hızlı ve etkili tedavi gerektirir. Günümüzde sterilizasyon şartlarının iyileşmesi ve antibiyotik proflaksisiyle endoftalmi sıklığında azalma olmasına rağmen göz içi cerrahi girişim sayısında artmadan dolayı total endoftalmi sayısında paralel bir azalmadan söz edilemez. Endoftalmiyle mü-cadelede temel kural, oluşmasını önlemektir. Bu amaçla hastanın hazırlan-masından, personel eğitimine, cerrahi aletlerin sterilizasyonu, riskli hastaların proflaksisi ve post operatif yakın takibi bir bütün olarak ele alınmalıdır. Cerrahi öncesi blefarit, konjonktivit ve dakriyosistit gibi enfeksiyöz hastalıkların tedavisi, cilt ve konjontivanın povidin iyot gibi antiseptik solüsyolarla temizlenmesi, kaş ve kirpiklerin örtülmesi için drape kullanılmasının endoftalmi riskini azalttığı gösterilmiştir. Cerrahi esnasında antibiyotik proflaksisi irrigasyon sıvısına antibiyotik konulması, subkonjonktival, intrakameral ve yara yerinin antibiyotikli irrigasyon sıvısıyla hidrate edilmesi şeklinde uygulanmaktadır. Bu yöntemler içinde en etkili olanı subkonjonktival ya da intrakameral antibiyotik uygulanmasıdır. Klinik endoftalmi tanısı konulan hastalarda kültür sonucu çı-kıncaya kadar olası etkenlerin tamamını kapsayabilecek intravitreal, subkonjonktival ve güçlendirilmiş topikal antibiyotik tedavisi başlanmalıdır. Sistemik antibiyotik kullanımı tartışmalı olsa da, ekstra oküler yayılımı kontrol altına almak amacıyla tedaviye ilave edilebilir.

Today, although improved sterilization conditions and antibiotic prophylaxis reduce the incidence of endophthalmitis, the amount of endophthalmitis hasn't decreased due to the increasing number of intraocular surgery. The most essential rule for fight with endophthalmitis is to prevent. From the preparation of the patient to staff training, sterilization of surgical instruments, close follow up of high risk patients and postoperative prophylaxis should be considered as a whole. Prophylactic treatment of blepharitis, conjunctivitis and dacryocystitis, cleaning of skin and conjunctiva with an antiseptic solution such as povidone iodine and to use a drape for covering eyebrows and eyelashes reduce the risk of endophthalmitis. Antibiotic prophylaxis during surgery is applied as an introduction in irrigation fluid, subconjunctival and intracameral. The most effective methods are subconjunctival or intracameral antibiotics application. Intravitreal, subconjunctival and topical fortified antibiotic therapy should be initiated as include all possible microbial agents in patients with clinical diagnosis of endophthalmitis until culture results. Systemic use of antibiotics is controversial, but it may be added to treatment in order to control of extraocular spread.
