“…The change in ocular flora due to chronic use of topical steroids in keratoconus patients with VKC can lead to an increased risk of postoperative keratitis [24]. Hence, while using ACXL to treat children, we recommend the usage of a limbal guard to protect the LSCs from damage and stoppage of the use of topical steroids (once the VKC is controlled) for at least two weeks prior to the procedure.…”

“…41,42 Antibacterial honeys also seem to stimulate monocytes in cell culture to release cytokines, tumor necrosis factor-a, interleukin-1, and interleukin-6, which activate the immune response to infection. 43 Both topical and systemic antibiotics can significantly reduce the CFUs in normal eyes 26,44 and in eyes with chronic lid disease, 24 but these effects are only short term, and the CFUs eventually return to pretreatment levels. 44 Dry eye conditions such as tear deficiency and meibomian gland disease are chronic conditions for which there is no cure.…”

Repeated dosing with topical or systemic antibiotics to control lid bacteria is expected to create resistance problems, and there are additional side effects to consider with repeated or long-term antibiotic dosing such as ocular surface toxicity with topical antibiotics and systemic side effects of oral agents.…”

“…Subgroup analysis demonstrates this is an independent risk factor and other potential confounding factors are not responsible (such as recent surgery or uveitis which are common causes for necessitating preoperative topical steroids). Topical steroids in the short term do not increase conjunctival bacterial flora counts,25 but topical steroids do reduce the function of leucocytes to oppose infection so we speculate that preoperative topical steroids may locally immunocompromise the eye and hence increase the risk of endophthalmitis.…”
