Osteonecrosis of the femoral head (ONFH) affects patients' quality of life. In our institute, some patients in the post-collapse state denied having an operation and considered continuing conservative treatment. Hence, the purpose of this study was to compare patient characteristics between patients who had total hip arthroplasty and those still on conservative treatment. We collected the data of seventy patients who had osteonecrosis of the femoral head with femoral head collapse from the hospital electronic database. All patients underwent the same approach in consideration of THA. We advised patients to shift to a THA treatment plan from a conservative one if the pain did not decrease by conservative treatment or improvement did not meet the patient's lifestyle and daily living requirements. We recorded the patient's demographic data, physical examination and hip radiographic. Twentynine patients (40 hips) had THA, while 31 patients (45 hips) were still undergoing conservative treatment. There were no differences in the patients' base demographic data. However, patients with THA had a lower rate of history of previous core decompression before head collapse than patients still undergoing conservative treatment (90.3% and 37.9%, respectively, p<0.01). In the conservative treatment group, most cases had femoral head collapse at the superolateral head, while patients in the THA group had an increased number of patients with collapse at the superomedial part of the femoral head. This study revealed that patients considering the continuation of conservative treatment had a higher rate of history of core decompression and a lower rate of supermedial femoral head collapse. Thus, these factors might be considered in the discussions with patients concerning the prognosis of the disease.

Comorbidities among Egyptian systemic lupus erythematosus: The COMOSLE-EGYPT study

Objective To study the prevalence and impact of comorbidities among a cohort of patients with systemic lupus erythematosus (SLE). Methods This study is retrospective, multicenter including 902 Egyptian patients with SLE. Medical records were reviewed for demographic data, clinical characteristics, routine laboratory findings, immunological profile, and medications. Moreover, SLE Disease Activity Index (SLEDAI), and the Systemic Lupus International Collaborating Clinics/American College Rheumatology Damage Index scores were calculated. Results Comorbidities were found in 75.5% of the studied group with hypertension and dyslipidemia as the most frequent comorbidities (43.1% and 40.1%, respectively), followed by sicca features, avascular necrosis, diabetes, osteoporosis and renal failure (11.5%,9%, 9%,8.9%, and 7.1%, respectively). Multivariate regression model showed statistically significant relation between the presence of comorbid condition and each of age ( P = 0.006), disease duration ( P = 0.041), SLEDAI at onset ( P < 0.001), cyclophosphamide intake ( P = 0.001), and cumulative pulse intravenous methylprednisone ( P < 0.001). Also, when adjusted to age and sex, those with multiple comorbid conditions had 18.5 increased odds of mortality compared to those without comorbidities (odds ratio (OR), 95% confidence interval (CI) = 18.5 (6.65–51.69)]. Conclusion Patients with SLE suffer from several comorbidities, with an increasing risk with age, longer disease duration, higher SLEDAI at onset, cyclophosphamide intake and cumulative pulse intravenous methylprednisone. Risk of mortality is exponentiated with multiple comorbidities.

This retrospective study analyzed the effectiveness of neuromuscular electrical stimulation (NMES) for pain relief caused by necrosis of femoral head (NFH).Totally, 80 cases of patients with NFH were analyzed and then were assigned to a treatment group or a control group in this study. Of these, 40 cases in the treatment group received ibuprofen and NMES therapy. The other 40 cases in the control group received ibuprofen alone. Cases in both groups were treated for a total of 6 weeks. The primary outcome of pain intensity was measured by a visual analog scale (VAS). The secondary outcome was assessed by Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC). In addition, adverse events (AEs) were also recorded in each case. All outcomes were evaluated before and after the treatment.After treatment, patients in the treatment group showed more effectiveness in pain relief, as measured by VAS (P < .01) and WOMAC sub-pain scale (P < .01), except stiffness, as evaluated by WOMAC sub-stiffness scale (P = .07), and function, as assessed by WOMAC sub-function scale (P = .09), than patients in the control group. Additionally, no significant differences in AEs were detected between 2 groups.This study found that NMES may be helpful for pain relief in patients with NFH.
