Background This double‐blind, randomized, controlled clinical trial assessed the efficacy of multiple sessions of antimicrobial photodynamic therapy (aPDT) as an adjunct to surgical periodontal treatment (ST) in patients with severe chronic periodontitis (SCP). Methods Sixteen patients with SCP were treated with aPDT+ST (test group, TG) or ST only (control group, CG), in a split‐mouth design. aPDT was applied at 0, 2, 7, and 14 postoperative days only in TG. All patients were followed up for 90 days after surgery. The following clinical and microbiological parameters were assessed: clinical attachment level (CAL), probing depth (PD), gingival recession (GR), bleeding on probing (BOP), plaque index (PI), and count of 40 subgingival microbial species (checkerboard DNA‐DNA hybridization). Data were collected at baseline (preintervention), at 60 days (30 days after the end of non‐surgical therapy), and at 150 days (90 days after surgery). Results A significant reduction in PD was observed at 150 days for the TG, when compared with the CG (P ˂ 0.05). CAL gain was significantly higher in the TG at 60 and 150 days (P ˂ 0.05). Changes in the subgingival microbiota were similar between the groups (P ˃ 0.05), but the TG revealed a larger number of bacteria associated with periodontal disease at the end of the experiment compared with the CG (P < 0.05). Conclusion Multiple sessions of aPDT as an adjunct to surgical periodontal treatment significantly improved clinical parameters at 90 postoperative days.

Learning from clinical phenotypes: Low‐dose biophotonics therapies in oral diseases

This narrative review on the use of biophotonics therapies for management of oral diseases is written as a tribute to Prof. Crispian Scully. His seminal contributions to the field are highlighted by the detailed, comprehensive description of clinical presentations of oral diseases. This has enabled a more thorough, fundamental understanding of many of these pathologies by research from his group as well as inspired mechanistic investigations in many groups globally. In the same vein, a major emphasis of this narrative review is to focus on the evidence from human case reports rather than in vitro or in vivo animal studies that showcases the growing and broad impact of biophotonics therapies. The similarities and differences between two distinct forms of low-dose biophotonics treatments namely photodynamic therapy and photobiomodulation therapy are discussed. As evident in this review, a majority of these reports provide promising evidence for their clinical efficacy. However, a lack of adequate technical details, precise biological rationale, and limited outcome measures limits the current utility of these treatments. Future investigations should attempt to address these shortcomings and develop better designed, rigorous, controlled studies to fully harness the tremendous potential of low-dose biophotonics therapies.

The oral maintenance of patients with periodontal disease mainly depends on clinical examination. However, insufficient number of medical workers cannot carry out detailed oral health education for a large number of patients within limited time and provide these patients with proper and effective oral health nursing methods. In this research, our study put forward a new Artificial Intelligence (AI) based method to diagnose chronic gingivitis, which is based on multichannel gray-levelco-occurrence matrix (MGLCM) and particle swarm optimization neural network(PSONN). Meanwhile, different training algorithms were used as comparison groups.The data set contains 800 images: 400 chronic gingivitis images and 400 healthy gingiva images. The results certify that the specificity, sensitivity, precision, accuracy and F1 Score of MGLCM (PSONN as a classifier) method is 78.17%, 78.23%, 78.24% ,78.20%and 78.17%, respectively. The association of MGLCM and PSONN is more accurate and efficient than approaches: NBC, WN+SVM,ELM and CLAHE+ELM.
