Effect of rinsing with povidone–iodine on bacteraemia due to scaling: a randomized‐controlled trial

Abstract: Rinsing with 7.5% povidone-iodine reduced the incidence and magnitude of bacteraemia and eliminated viridans streptococci from such bacteraemia. Povidone-iodine rinsing may be helpful for ultrasonic scaling of gingivitis patients at risk of infective endocarditis.

“…Various degrees of bacterial dissemination are associated with periodontal probing (27,28), scaling (22,68), root planing, and subgingival irrigation (5,77,106,141). The presence of inflammation in the periodontal site is an important contributory factor to bacterial dissemination, as shown by the increased incidence of bacteremia in subjects with periodontitis compared to those with gingivitis (where the degree of inflammation is considered less severe) and those with healthy periodontium (48).…”

“…Invasive dental procedures that traumatise the periodontium such as periodontal probing, scaling and root planing, subgingival irrigation, periodontal surgery, rubber dam placement, matrix band placement and extractions can allow microbes in the gingival sulcus/pocket to gain entry into the systemic circulation and cause bacteraemia , Sconyers et al 1973, Berger et al 1974, Baumgartner et al 1976, Forner et al 2006, Cherry et al 2007. It is thought that the more invasive and the longer the duration of the procedure, the greater the incidence of bacteraemia detected (Lockhart 2000).…”

“…The intensity of bacteraemia necessary to induce experimental infective endocarditis in animals is of the order of 1 x 10 6 to 1 x 10 7 colony forming units (CFU) per mL (Bahn et al 1978). A dentally associated bacteraemia in adult humans is usually of a low-grade intensity and of the order of <10 CFU per mL of blood , Forner et al 2006, Cherry et al 2007). …”
