Effect of radiotherapy after breast-conserving surgery in older patients with early breast cancer and breast ductal carcinoma in situ: a meta-analysis

Abstract: BackgroundThere are no consistent agreements on whether radiotherapy after breast-conserving surgery (BCS) could provide local control and survival benefit for older patients with early breast cancer or breast ductal carcinoma in situ (DCIS). The present study aimed to evaluate the efficacy of radiotherapy after BCS in older patients with early breast cancer or DCIS.ResultsRadiotherapy could reduce the risk of local relapse in older patients with early breast cancer. The 5-year AR of local relapse was 2.2% and… Show more

“…All the studies differed conceptually including not only elderly patients but also different tumor sizes or systemic therapies. In summary, no significant differences have been shown regarding OS but some decreases in in-breast recurrences after RT [27][28][29] as recent meta-analyses confirmed [30,31]. Interestingly, there is also some evidence that effects of RT also depend on histopathologic subtypes since RT did not improve OS in patients with invasive lobular or uncommon invasive breast cancer types [32].…”

“…Of the 10 studies, all had surgery, 5 had radiotherapy [ 47 , 49 , 51 , 52 , 53 ], 5 had chemotherapy [ 46 , 49 , 52 , 53 , 54 ], and 2 had hormone therapy [ 47 , 49 ]. Adjuvant radiotherapy is the standard treatment following breast conserving surgery in early breast cancer [ 58 ].…”

“…A recent meta-analysis also indicated that radiotherapy could slightly reduce the risk of local relapse in older patients if breast cancer was diagnosed early. However, reduced local relapse cannot translate into significant survival benefits (5). These previous studies have not provided uniform, positive results regarding radiotherapy for breast cancer patients.…”
