Effect of radiotherapy after breast-conserving surgery on 10-year recurrence and 15-year breast cancer death: meta-analysis of individual patient data for 10 801 women in 17 randomised trials

Abstract: SummaryBackgroundAfter breast-conserving surgery, radiotherapy reduces recurrence and breast cancer death, but it may do so more for some groups of women than for others. We describe the absolute magnitude of these reductions according to various prognostic and other patient characteristics, and relate the absolute reduction in 15-year risk of breast cancer death to the absolute reduction in 10-year recurrence risk.MethodsWe undertook a meta-analysis of individual patient data for 10 801 women in 17 randomised… Show more

“…37 In addition to its direct tumoricidal effects, radiotherapy is known to induce distinct changes in the tumor microenvironment, particularly in the leukocyte compartment. 38 Meanwhile, it is well acknowledged that anti-tumor immune mechanisms can be stimulated by radiotherapy in the sense of an in situ cancer vaccination.…”

“…Adjuvant radiotherapy (RT) for breast cancer is a standard treatment used to improve local tumor control and overall survival 1, 2, 3, 4. Volumetric modulated arc therapy (VMAT) has been evaluated for breast treatment in several publications as attested in a recent review 5.…”

“…Adjuvant chemoradiotherapy after breast‐conserving surgery cannot only reduce local recurrence risk effectively but also decrease distant metastasis rate and increase survival rate and quality of life of patients significantly 17, 18. Thus, the same curative effect with radial operation or modified radical operation is achieved.…”
