DIETER SOYKA | MICHAEL SOYKA β β-Adrenozeptorblocker in der Neurologie β β-Adrenozeptorblocker und Migräne Die Ergebnisse erster systematischer Therapiestudien mit Propranolol, bei denen nach einer Reduzierung der Migräneattackenfrequenz gefragt wurde, waren durchweg überzeugend [2-7]. Nach der Entwicklung weiterer β-Rezeptorenblocker wurden einschlägige Therapiestudien auch auf Während der klinischen Anwendung von Propranolol bei kardiovaskulären Indikationen wie arterieller Hypertonie und Angina pectoris gaben einige Patienten, bei denen eine Komorbidität mit Migräne bestand, eine deutliche Reduzierung ihrer Migräneattackenfrequenz an [1], Anlass für erste systematische Therapiestudien mit Propranolol bei Migräne. diese neuen Substanzen ausgedehnt, aber nun zeigten sich uneinheitliche Ergebnisse. Bei einigen β-Blockern war der therapeutische Effekt mit dem des Propranolols vergleichbar, andere Substanzen waren nicht oder schwächer migränewirksam. Neben Propranolol wurde besonders Metoprolol in zahlreichen, doppelblind Placebo-kontrollierten Studien untersucht und erwies sich dem Propranolol ebenbürtig [8-11], dabei in der Verträglichkeit vielleicht noch etwas günstiger. Als weitere migränewirksame β-Blocker wurden Atenolol, Timolol, bis zu einem gewissen Grade auch Pindolol, Practolol und Bisoprolol identifiziert [5, 8, 10-20]. Als nicht wirksam erwiesen sich Nadolol, Alprenolol, Oxprenolol und Sotalol. Die Migränewirksamkeit der genannten Substanzen war letztlich so beeindruckend, dass sie bald in den Therapieempfehlungen der Deutschen Migräne-und Kopfschmerzgesellschaft (DMKG) als Mittel der ersten Wahl für die Migräneprophylaxe (nicht für die Attackenkupierung!) eingestuft wurden, und es zeigte sich diesbezüglich auch international eine einheitliche Bewertung. Sie gilt noch heute [21], auch wenn sich inzwischen das Repertoire an Pharmaka zur Migräneprophylaxe erfreulich erweitert hat, vor allem mit dem Calcium-Antagonisten Flunarizin [22, 23], dem vasoaktiven Cyclandelat und der als Antikonvulsivum bekannten Valproinsäure [22]. Der Einsatz von β-Blockern wird empfohlen, wenn über mehrere Monate hinweg mehr als zwei Attacken monatlich auftreten oder die einzelnen Attacken sehr schwer verlaufen, weil sie nicht auf die modernen Triptane ansprechen oder diese nicht vertragen werden [24]. Bemerkenswert, nicht zuletzt im Hinblick auf den möglichen Wirkungsmechanismus (s. weiter unten), ist die Beobachtung, dass der erwünschte Therapieerfolg mit β-Blockern, also die Reduzierung der Attackenfrequenz und -stärke, erst nach ca. 4 bis 6 Wochen deutlich wird. Gerade auf den verzögerten Therapieerfolg müssen die Patienten hingewiesen werden, um einem vorzeitigen Therapieabbruch vorzubeugen. Ungeklärt ist die Frage, warum eine Gruppe von Migränepatienten auf β-Blocker nicht anspricht [22, 25, 26]. Die Zahl der Non-Responder beträgt ca 20 bis 30 %, was übrigens auch für die Therapie mit anderen medikamentösen und nichtmedikamentösen Therapieverfahren gilt.
