Effect of Plasma Osmolality on Pituitary-Adrenal Responses to Corticotropin-Releasing Hormone and Atrial Natriuretic Peptide Changes in Central Diabetes Insipidus

Abstract: The objective of the present study was to examine the effect of changes in plasma osmolality (pOsm) on the responses of the pituitary-adrenal axis to CRH and atrial natriuretic peptide (ANP) release in patients with central diabetes insipidus (DI). Eight normal subjects and six DI patients were subjected to human CRH (hCRH) (1 microgram/kg) stimulation alone or associated with isotonic volume loading (0.9% NaCl, 12 mL.kg.60 min) or an osmotic stimulus (5% NaCl, 0.06 mL.kg/min.120 min). The DI group showed sign… Show more

“…Growth retardation is also observed in patients with nephrogenic DI (14,23). Several factors could predispose patients with untreated DI to growth failure, such as decreased calorie intake due to predominant fluid consumption or loss of appetite because of an activated hypothalamic-pituitary axis (HPA) stress axis in chronic hyperosmolality (24). Upon treatment of the AVP deficiency by DDAVP, a catch-up growth over several months occurred in all our patients.…”

“…Plasma levels of ANP, OT, and VP were measured by radioimmunoassay as previously described (16,23,24). VP and OT were extracted from 1 ml of plasma with acetone and petroleum ether, and ANP was extracted from 1 ml of plasma using Sep-Pak C-18 cartridges (Waters, Milford, MA).…”

“…AVP concentration was measured by radioimmunoassay (RIA) after extraction of plasma (1 mL) with acetone and petroleum ether, at the Laboratory of Endocrine Physiology, Ribeirão Preto School of Medicine, University of São Paulo, as previously reported [18]. The percentage of recovery after extraction was 83% (ranging from 50% to 99%).…”
