Background: To investigate the effects and mechanism of high concentration glucose (HG), exogenous hydrogen peroxide (H 2 O 2 ), and antioxidants on the cell growth (cell proliferation) of human peritoneal mesothelial cells (HPMCs).Methods: All tests were conducted on cultured HPMCs (HMrSV5) in vitro. Various concentrations of glucose (0.1%, 1.35%, and 3.86%), H 2 O 2 (0.5 and 0.1 mmol/L), and antioxidants (pyruvate and catalase) were used in cell culture. Moreover, in order to study the interaction between these factors, HG and H 2 O 2 , HG and antioxidants, HG, H 2 O 2 , and antioxidants, were used respectively. After 12-24 h, phase-contrast microscopy was used to examine the morphological changes of HPMCs. DNA synthesis was detected by 3 H-thymidine incorporation to measure cell proliferation, and flow cytometry was used to evaluate the proportion of cells in G 1 phase. Furthermore, semiquantitative reverse-transcription polymerase chain reaction (RT-PCR) was utilized to determine the mRNA expression of p21 Waf1 and p27 Kip1 [cyclin-dependent kinase inhibitors (CKIs)], while immunocytochemistry (ICC) and Western blotting were employed to measure the protein expression of p21 Waf1 and p27 Kip1 .Results: HG or low-dose exogenous H 2 O 2 resulted in hypertrophy and senescence of HPMCs, resulting in similar morphological changes. Both HG and exogenous H 2 O 2 (0.5 mmol/L) inhibited the proliferation of HPMCs and led to G1 phase arrest of HPMCs. The proportion of cells in G 1 phase increased. Moreover, HG enhanced the toxic effects of exogenous H 2 O 2 . Both HG and exogenous H 2 O 2 increased the expression of p21 Waf1 and p27 Kip1 . The addition of an antioxidant in HG medium arrested cells in the G 1 phase and improved the inhibited cell proliferation.Conclusions: Both HG and exogenous H 2 O 2 treatments can induce growth inhibition of HPMCs by arresting cell cycle progression, which is partially due to the increased expression of p21 Waf1 and p27 Kip1 . Thus, the effects of HG might be associated with endogenous reactive oxygen species (ROS), and it might be beneficial to use antioxidants in peritoneal dialysis (PD).

Background: The beneficial effects of pyruvate on lactic acidosis and energy metabolism have been studied. The ability to attenuate acidosis and improve aerobic system contribution are essential for team sports athletes to perform multiple sprints in a limited time. This study aimed to investigate the impact of pyruvate supplementation on energy metabolism and lactic acidosis during high-intensity interval exercise (HIIE) and to evaluate its role in repeated sprint exercise (RSE) performance.Methods: Fourteen national-level male soccer athletes from China Football College (age: 20 ± 2 years, body fat: 13.11 ± 3.50%) participated in a randomized, double-blind, crossover study. Each subject underwent two HIIE and RSE tests after one week of supplementation with 0.1g/kg/d of pyruvate or placebo. Venous blood gas (pH, bicarbonate (HCO3-), base excess (BE) and oxygen partial pressure (pO2)) were measured at baseline, pre-HIIE, post-HIIE, pre-RSE and post-RSE. Finger-stick capillary lactate were collected at baseline, immediately after each bout of HIIE, and 3, 5, 7 and 10 min post HIIE. The energy system contributions during HIIE and exercise performance of RSE were analyzed. Results: Blood pH (p < 0.01), HCO3- (p < 0.01) and BE (p < 0.01) were significantly lower than their baseline levels at post-HIIE, pre-RSE and post-RSE in both Group pyruvate (PYR) and Group placebo (PLA), respectively. Blood pH, HCO3- and BE were significantly improved in PYR at pre-HIIE (p < 0.01), post-HIIE (p < 0.01) and pre-RSE (p < 0.01) than in PLA. Furthermore, blood BE remained higher in PYR than in PLA until the end of RSE (p < 0.05). Blood pO2 was higher at post-HIIE (p < 0.05), pre-RSE (p < 0.01) and post-RSE (p < 0.01), compared to baseline, in two groups and further higher in PYR than in PLA at post-HIIE (p < 0.05) and post-RSE (p < 0.05). During HIIE, the contribution of the aerobic system in the fourth bout of HIIE was higher in PYR than in PLA (p < 0.05). The contribution of the anaerobic energy was higher than that of the aerobic energy in PLA (p < 0.01), but without difference in PYR (p > 0.05). In addition, compared with PLA, the relative peak power (RPP) of the first and fifth sprints, the relative average power (RAP) of the fifth sprint and the average of RPP and RAP during RSE were significantly improved in PYR (p < 0.05). However, the blood lactate (p > 0.05), PD% of each bout (p > 0.05) or average PD% of RSE (p > 0.05) showed no significant difference between the two groups. Conclusion: Pyruvate supplementation with 0.1g/kg/d for a 1-week enhances aerobic metabolism and buffers exercise-induced metabolic acidosis but not lactic acidosis during HIIE, improving RSE performance in moderate acidosis. Thus, after 1 week pyruvate supplementation, athletes are expected to balance aerobic and anaerobic energy contributions and attenuate metabolic acidosis during team sports, improving exercise performance.

Pyruvate as a Potential Beneficial Anion in Resuscitation Fluids

There have been ongoing debates about resuscitation fluids because each of the current fluids has its own disadvantages. The debates essentially reflect an embarrassing clinical status quo that all fluids are not quite ideal in most clinical settings. Therefore, a novel fluid that overcomes the limitations of most fluids is necessary for most patients, particularly diabetic and older patients. Pyruvate is a natural potent antioxidant/nitrosative and anti-inflammatory agent. Exogenous pyruvate as an alkalizer can increase cellular hypoxia and anoxia tolerance with the preservation of classic glycolytic pathways and the reactivation of pyruvate dehydrogenase activity to promote oxidative metabolism and reverse the Warburg effect, robustly preventing and treating hypoxic lactic acidosis, which is one of the fatal complications in critically ill patients. In animal studies and clinical reports, pyruvate has been shown to play a protective role in multi-organ functions, especially the heart, brain, kidney, and intestine, demonstrating a great potential to improve patient survival. Pyruvate-enriched fluids including crystalloids and colloids and oral rehydration solution (ORS) may be ideal due to the unique beneficial properties of pyruvate relative to anions in contemporary existing fluids, such as acetate, bicarbonate, chloride, citrate, lactate, and even malate. Preclinical studies have demonstrated that pyruvate-enriched saline is superior to 0.9% sodium chloride. Moreover, pyruvate-enriched Ringer’s solution is advantageous over lactated Ringer’s solution. Furthermore, pyruvate as a carrier in colloids, such as hydroxyethyl starch 130/0.4, is more beneficial than its commercial counterparts. Similarly, pyruvate-enriched ORS is more favorable than WHO-ORS in organ protection and shock resuscitation. It is critical that attention first be paid to improving abnormal saline with pyruvate for ICU patients. Many clinical trials with a high dose of intravenous or oral pyruvate were conducted over the past half century, and results indicated its effectiveness and safety in humans. The long-term instability of pyruvate aqueous solutions and para-pyruvate cytotoxicity is not a barrier to the pharmaceutical manufacturing of pyruvate-enriched fluids for ICU patients. Clinical trials with sodium pyruvate-enriched solutions are urgently warranted.
