Effect of periodontal ligament removal with gauze prior to delayed replantation in rabbit incisors on rate of replacement resorption

Abstract: Removal of PDL prior to delayed replantation may result in some initial protection of the cementum during the first few weeks. However, over longer times there seems to be neither protection of the dentin from ankylosis and osseous replacement, nor any influence on the rate of replacement resorption.

“…The results of the present study, showing that 15% of teeth remain non‐ankylotic after more than one‐hour of dry storage, raise some doubt as to whether PDL removal should in fact be carried out after the tooth has been stored for periods exceeding 60 minutes. Another study also questioned the benefit of PDL removal to delay replacement resorption, showing no influence of PDL removal on the rate of replacement resorption . If removal of the PDL is carried out, ankylosis will most likely occur.…”

“…Today, pulp space infection and its related root resorption can be prevented or treated successfully by proper endodontic treatment, halting its progression . If the avulsed tooth has been exposed to air for long periods before replantation, the PDL will become necrotic due to the drying process, so healing with a normal PDL is hindered and no longer possible . In such teeth, the necrotic PDL will allow bone ingrowth, and the tooth will gradually be substituted by bone .…”
