Effect of Perindopril on Cerebral Vasomotor Reactivity in Patients With Lacunar Infarction

Abstract: Background and Purpose-There is growing evidence that pharmacologic interference with the renin-angiotensin system may reduce risk of stroke, although the mechanism is unclear. Impaired reactivity of cerebral vessels has recently been recognized as a risk factor for stroke. We examined the effect of the angiotensin-converting enzyme (ACE) inhibitor perindopril on cerebral vasomotor reactivity to acetazolamide in patients with lacunar cerebral infarction. Methods-We studied a cohort of male patients between 3 a… Show more

“…Hand-searching identified 3 additional publications. Of the 30 for full text reading, 3 were excluded (unable to translate: Russian 16 and Slovakian 17,18 ) and 11 failed to meet the inclusion criteria (no control subjects, 19,20 not assessing dynamic endothelial function, or inappropriate patients [21][22][23][24][25][26][27][28][29] ). Therefore, 16 papers were eligible, including 974 individuals.…”

“…Both classes of drugs affect the modulation of the vasodilation properties of cerebral vessels and are able to induce a significant improvement of cerebral hemodynamics. 28,29 The latter aspect deserves further attention in terms of the scope for developing novel therapeutic strategies, at least in a subgroup of AD patients with impaired cerebral hemodynamics. “…Hypertension is one of the risk factors for impediment in many internal organs, for example, the brain, heart, kidney, blood vessels, and the retina (25)(26)(27)(28)(29)(30)(31). Several studies have discussed the role of hypertension as a renal prognostic factor (27)(28)(29), while others (32,33) (35).…”
