Effect of PEEP, blood volume, and inspiratory hold maneuvers on venous return

Abstract: According to Guyton's model of circulation, mean systemic filling pressure (MSFP), right atrial pressure (RAP), and resistance to venous return (RVR) determine venous return. MSFP has been estimated from inspiratory hold-induced changes in RAP and blood flow. We studied the effect of positive end-expiratory pressure (PEEP) and blood volume on venous return and MSFP in pigs. MSFP was measured by balloon occlusion of the right atrium (MSFPRAO), and the MSFP obtained via extrapolation of pressure-flow relationshi… Show more

“…Eleven pigs were included in the study. As previously described (4), after premedication with intramuscular ketamine (20 mg/kg) and xylazine (2 mg/kg), induction and intubation followed with midazolam (0.5 mg/kg). Anesthesia was maintained with propofol (4 mg·kg Ϫ1 ·h Ϫ1 ) and fentanyl (5 g·kg Ϫ1 ·h Ϫ1 ), and the depth was controlled by repeatedly testing the response to nose pinch.…”

“…Flow was resumed after 30 s or earlier if signs of a reflex-mediated increase in arterial blood pressure (ABP) were seen. MSFP was taken as the mean value of RAP during three beats of equilibrium defined from ABP nadir (4). At least 3 min were allowed for heart rate and blood pressure to return to baseline.…”

“…On the other side, an increase in thoracic pressure due to MV [9] may have an effect on inflammation and edema in the abdominal compartment. In fact, the decrease in venous return related to high positive end-expiratory pressure (PEEP) ventilation [10] has been well described. In addition, a reduction on portal flow due to a vasoconstrictive reaction related directly to PEEP [11] has also been described.…”

“…Modern concepts of invasive MV mainly involve different modes of positive pressure ventilation alongside with oxygen supply, which induce several changes to the patient's cardiovascular system (Pinsky 2005;Frazier 2008). The transition from negative pleural pressure throughout the spontaneous breath cycle to nonphysiological positive intrathoracic pressure under MV compromises venous blood return to the right atrium (Pinsky 2005;Berger et al 2016). Furthermore, hyperinflation of alveoli and compression of small pulmonary vessels increase pulmonary vascular resistance (PVR) and right ventricular afterload leading to elevated right atrial pressure, myocardial stretch, workload, and oxygen demand (Cherpanath et al 2016).…”
