“…The nonabsorbable antibiotic neomycin has been used prophylactically in the treatment of cirrhotic patients with hepatic encephalopathy with minimal side effects (71) and might be considered for prophylactic SDG. However, neomycin reduces the level of both aerobes and anaerobes (with potential loss of colonization resistance); reduces aerobic flora temporarily; and has otic and renal side effects, depending on the cumulative dose (21, [25][26][27][28][29][30]72). An acceptable prophylactic agent must (a) reduce the aerobic gramnegative flora, particularly E. coli, for as long as the antibiotic is given (i.e., no resistant strains); (b) have minimal effects on the anaerobic flora; (c) have minimal toxicity even with liver failure; (d) prevent SBP; (el be more effective than the current means of detection and treatment of SBP; and (f) be cost-effective.…”
