Effect of Optic Nerve Disinsertion During Evisceration on Nonporous Implant Migration: A Comparative Case Series and a Review of Literature

Abstract: The rate of nonporous implant migration due to optic nerve disinsertion is not statistically or clinically significant in evisceration with optic nerve disinsertion, allowing placement of a larger implant and fabrication of a custom ocular prosthesis with an ideal weight.

“…Migration was classified as decentration and displacement based on our previous work. [ 9 ] All six patients had an inferotemporal displacement of the implant with the implant palpable anterior to the inferior orbital rim and consequent shallowing of the inferior fornix. They had inability to retain a custom ocular prosthesis (COP) or achieve a satisfactory centration of the prosthesis after maximal prosthesis modification by an expert ocularist.…”

“…This technique allows for the placement of larger orbital implants and helps in reducing the weight of the prosthesis; migration tends to be lower with porous implants, although nonporous implants can be a reasonable alternative for lower cost. 46 …”

“…[ 2 ] This makes it important for us to understand the concept of making scleral flaps during evisceration and ideal sizing of implants to achieve a symmetric fullness of the superior sulcus and avoid anophthalmic ptosis. [ 3 ] Along with this, one of the most important factors that is sub-optimally addressed is the cosmetic outcome of the prosthesis itself. [ 4 ] This puts an emphasis on the development of a metric to assess the aesthetic outcome of an anophthalmic socket incorporating all of these factors.…”
