Effect of manual physiotherapy in homogeneous individuals with subacromial shoulder impingement: A randomized controlled trial

Abstract: Objective To compare the effect of specific interventions aimed at (1) the upper thoracic spine (passive mobilization) and (2) the posterior shoulder (massage, passive mobilization, and stretching) to (3) an active control intervention in a homogeneous group with extrinsic subacromial shoulder impingement (SSI). Study Design Single‐centre, prospective, double‐blinded, randomized controlled trial. Method Eligible individuals with clearly defined extrinsic SSI were randomized to each group. Treatment duration wa… Show more

“…24 The mitigation of shoulder pain, which paralleled the improvement in function, prepared the patient for rehabilitative exercise: manual therapy reduces the signs and symptoms of extrinsic subacromial shoulder impingement more if the pain before rehabilitative treatment is not high. 25 The innovative technical characteristics of CARE therapy allow the transfer of high energy levels without causing significant temperature increases, thus achieving excellent results, even for acute pain associated with shoulder impingement syndrome. The typical characteristics of shoulder impingement in Neer stage I are reversible lesions with oedema and haemorrhage.…”

“…50,74,81 Grade III or IV joint mobilizations 37 were preferentially used to reduce hypomobility of the posterior capsule and surrounding tissue, improve glenohumeral arthrokinematics, and reduce symptoms. 38,44,51 Exercises and stretching were initially taught, supervised, and gradually progressed by the treating therapist, in conjunction with the stretching exercises and "phase 1" strengthening. 81 In addition, this group also received 8 to 15 minutes of soft tissue mobilization targeting the posterior and anterolateral shoulder region.…”

“…Forward shoulder posture causes shoulder muscle imbalance and is a risk factor for shoulder dysfunction (Yoo, 2013). Thus, thoracic mobilization can change shoulder alignment, which may affect shoulder dysfunction (Cho et al, 2017;Land et al, 2019;Park et al, 2020).…”
