Effect of latissimus dorsi flap breast reconstruction on the strength profile of the upper extremity

Abstract: Because no significant loss of contra-movement torque strength was observed, it was concluded that the loss of synergistic torque strength appears to result from the loss of LD function per se.

“…Additionally, it proved that DASH results were lower for patients engaged in sports activities, especially those using the latissimus dorsi [27]. Research by Huizum et al also showed the significant difference of DASH results between post-LDBR patients and healthy subjects, which additionally correlated with the upper limb muscle strength decrease [28]. Other authors point out the fact of upper limb muscle strength decrease as well [10,29,30].…”

“…A number of authors have used the Disability of the Arm, Shoulder and Hand Questionnaire (DASH), to evaluate shoulder function following LD flap breast reconstruction with study design and followup varying from study to study (Bonomi et al, 2012;Brackley et al, 2010;Button, Scott, Taghizadeh, Weiler-Mithoff, & Hart, 2010;Garusi et al, 2016;Glassey et al, 2008;Koh & Morrison, 2009;Paolini, Amoroso, Pugliese, Longo, & Santanelli, 2014;Saint-Cyr et al, 2009;van Huizum et al,2016;Yang et al, 2015). The DASH questionnaire consists of 30 items relating to ADL, work, sport and music, each question of which has five options for response.…”

“…Some authors have reported substantial morbidity following surgery, however, the length of disability sustained is inconsistent (Bonomi et al 2012;Button et al, 2010;Koh & Morrison, 2009;van Huizum et al, 2016), highlighting the lack of consensus within the literature. These timescales are identified as broad recommendations with the authors highlighting that timescales will vary on an individual case basis, and that all exercise prescription should be tailored to the individual patient.…”

“…Some types of reconstruction, such as the LD flap, cause decreased muscle strength compared with controls, while other types of reconstruction, such as those using implants or a mastectomy, can cause pectoralis tightness . In a systematic review by Hidding et al done in 2014, patients who underwent immediate breast reconstruction showed less impairment of upper extremity function compared with those who underwent only a mastectomy.…”
