Objectives: Heavy Menstrual Bleeding (HMB) is the most important problems of Uterine Leiomyoma (UL). This study aimed to assess the effect of the extract of the Capsella bursa-pastoris (CBP) on the control of HMB and quality of life in patients with uterine leiomyoma. Design: In a double-blind randomized, clinical trial 54 women with uterine leiomyoma were randomly assigned to the intervention/control groups by block randomization. Setting: Gynecology outpatient clinics. Intervention: The intervention group received 350 mg of alcoholic extract of Capsella bursa-pastoris and the control group received placebo twice daily for three months. Main Outcome Measures: Amount of bleeding by Pictorial blood loss assessment chart (PBAC), quality of life by menstrual quality of life questionnaires (MQ) and bleeding duration by calendar were evaluated. Results: The mean of PBAC decreased from 464.00± 283.61 at baseline to 323.82±207.66 in the intervention group and decreased from 445.92±362.64 to 214.36±137.68 in control group in the third month. The improvement trend was significant in the two groups, but there was no significant difference between groups. The mean of bleeding duration and menstrual quality of life showed improvement in patients of two groups without significant difference between the two groups. Conclusion: Despite the effectiveness of CBP in the intervention group in decreasing of PBAC score and menstrual bleeding duration and improvement of menstrual quality of life in patients who suffered from UL, it did not show a significant effect compared to the control group. Future studies with a larger sample size in one specific type of UL suggested.

Medicinal Plants as Potential Hemostatic Agents

Purpose: Medicinal plants with a variety of phytochemical ingredients remain a potential source for new drug discovery. The use of medicinal herbs in a wide range of diseases and symptoms, such as bleeding, is prevalent in traditional and ethno medicine worldwide. Thus, this work provides a comprehensive review of medicinal plants or their isolated compounds, with respect to their ethno-medicinal use, which have demonstrated the stimulating effect on the hemostasis process. Methods: The relevant studies were withdrawn from electronic databases including Pubmed, EMBASE and Web of Science with a structured search methodology. Results: The total of 17 medicinal plants with hemostatic activity were extracted. The most frequently studied plant families were Compositae, Lamiaceae, Fabaceae, and Asteraceae. Bioactive compounds exerting hemostatic activity included tannins, iridoid glycosides, glycoconjugate, lignan, saponins and phenolic compounds. The most attributed mechanisms include coagulation stimulation via increasing the factor XII activity and plasma fibrinogen levels, the fibrinolysis inhibition, vascular or smooth muscle constriction and platelet aggregation. The most important adverse effects of high dose extract or isolated compounds administration were hepatotoxicity and nephrotoxicity. Conclusion: This review provides a list of medicinal plants with hemostatic activity that could be used as valuable sources of new plant-based hemostatic agents. Furthermore, this could be practical in detecting possible interactions of plants with anticoagulant, antiplatelet, fibrinolytic and antifibrinolytic medications. __________________________________________________________________________________________

Background: Knee replacement surgery is often successful; however, a number of patients are not satisfied with the results of the operation. Objectives: This study was designed to determine whether clinical presentation and quality of life (QoL) before knee replacement surgery could predict patient satisfaction with surgery. Materials and Methods: A cross-sectional study was performed among 250 elderly patients with the age of 60 years or over who were candidate for a unilateral knee replacement surgery in hospitals affiliated to Shiraz University of Medical Sciences. To gather data, the QoL Questionnaire, knee evaluation form, demographic, and satisfaction questionnaire were used. The questionnaires were filled-in before and after surgery and in follow-up time (2 months later). Results: Most of the respondents were women (66.4%) with the mean age of 68. 9 (standard deviation = 6. 84). The results showed a significant positive correlation between patients' post-operative satisfaction and pre-operative clinical presentation (r = 0.195 P = 0.003) and QoL (r =156, P = 0.023). However, there was no significant association between post-operative satisfaction and pre-operative clinical presentation and QoL at follow-up time (P > 0.05). Linear regression analysis showed that pre-operation clinical presentations and QoL, respectively, explained 3.8% and 2.4% of the variance in post-operative satisfaction. Conclusions:The results showed that satisfaction after surgery in the elderly is partly influenced by the pre-operative QoL and clinical presentation. Therefore, for improvement of the patients' post-operative outcomes, it is necessary to pay more attention to these variables in future planning.
