SUMMARYThe use of chlorhexidine for caries prevention has been a controversial topic among dental educators and clinicians. In several reviews, it has been concluded that the most persistent reduction of mutans streptococci have been achieved by chlorhexidine varnishes, followed by gels and, lastly, mouth rinses. Also, the evidence for using different chlorhexidine modes or a combination of chlorhexidine-fluoride therapy for caries prevention has been "suggestive but incomplete." Variable study designs and lack of data in highrisk children and adults support the need to continue conducting randomized, well-controlled clinical trials and to search for a practical, effective mode of antimicrobial treatment that augments the known effect of fluoride treatments.Currently, the only chlorhexidine-containing products marketed in the United States (US) are mouthrinses containing 0.12 percent chlorhexidine. Based on the available reviews, chlorhexidine rinses have not been highly effective in preventing caries, or at least the clinical data are not convincing. Due to the current lack of long-term clinical evidence for caries prevention and reported side effects, chlorhexidine rinses should not be recommended for caries prevention. Due to the inconclusive literature and sparse clinical data on gels and varnishes, their use for caries prevention should also be studied further to develop evidence-based recommendations for their clinical role in caries prevention.Since dental caries is a disease with a multifactoral etiology, it is currently more appropriate to use other established, evidence-based prevention methods, such as fluoride applications, diet modifications and good oral hygiene practices. Recent findings also indicate that the effect of an antimicrobial agent for reducing the levels of mutans streptococci or plaque reduction may not always correlate with eventual caries reduction. The clinically important outcome is proven reductions in caries.Many advances in the treatment and prevention of dental caries have been introduced over the past century. Caries Prevention in a Community–Dwelling Older Population

A clinical trial was conducted to compare the effect of different caries–preventive strategies on caries progression in lower–income, ethnically diverse persons 60 years of age and older. Two hundred and ninety–seven subjects were randomized into one of five experimental groups. Group 1 received usual care from a public health department or a private practitioner. Group 2 received an educational program of 2 h duration implemented twice a year. Group 3 received the educational program plus a 0.12% chlorhexidine rinse weekly. Group 4 received the education and chlorhexidine interventions and a fluoride varnish application twice a year. Group 5 received all the above interventions as well as scaling and root planing every 6 months throughout the 3–year study. A carious event was defined as the onset of a carious lesion, a filling, or an extraction on a surface which was sound at baseline. Two hundred and one subjects remained in the study for the 3–year period. Groups that received usual intraoral procedures (groups 3, 4, and 5) had a 27% reduction for coronal caries events (p = 0.09) and 23% for root caries events (p = 0.15), when compared to the groups that received no intraoral procedures (groups 1 and 2). Routine preventive treatments may have had only a small–to–moderate effect upon caries development.

Plaque pH was studied during 60 min in situ in 10 subjects after eating various breads. The pH response was then compared to glycemic index (GI; calculated from the incremental blood glucose area) obtained from our earlier investigations. The following four products were tested: (1) barley kernel bread (BKB), (2) BKB, sourdough–fermented (BKBS), (3) white wheat bread (WWB), and (4) syrup–sweetened wheat–rye bread (SWRB). BKB was also tested with more intensive chewing and SWRB with the addition of fat. A 5% sucrose solution served as a control. The pH drops with all the breads were considerably smaller than with the sucrose solution during the first 15 min. From 30 min and onwards the breads gave similar, or even lower (SWRB) pH than sucrose. There was a great difference in pH response among the four breads, with the most pronounced pH fall for SWRB, followed by WWB (based on the AUC values). Intensive chewing of BKB increased, while the addition of fat to SWRB reduced the pH fall, in both cases by about 0.2 pH units. A high correlation (r = 0.94) between plaque pH and GI was found, i.e. the more pronounced the pH drop in plaque, the higher the GI in blood. Therefore, both from a cariogenic and from a metabolic point of view, breads with a low GI should be recommended.
