Effect of Desloratadine on Patients with Allergic Rhinitis and Exercise-Induced Bronchoconstriction: A Placebo Controlled Study

Abstract: Desloratadine does not have an effect on exercise induced bronchoconstriction. Patients with allergic rhinitis with exercise induced bronchoconstriction who are on desloratadine will still require treatment with beta(2) agonist or leukotriene receptor antagonist for their symptoms.

“…Some clinicians may opt to use anticholinergic inhaler therapy,32 and mast cell stabilising agents such as cromoglicate or nedocromil33 34 have also been used in the treatment of EIB 12. In patients with EIB and allergies, an antihistamine may also be helpful in improving overall asthma control but does not appear to have an effect on EIB 12 35. A regime that allows for moderate exercise during summer will not necessarily be effective or appropriate in an elite athlete competing in winter sports, and this should be considered when discussing treatment.…”

“…Although desloratadine treatment resulted in a significantly less drop in the FEV1 compared to the baseline visits, no statistical difference was observed in this regard between desloratadine and placebo 26 . Moreover, the area under the curve for percentage fall in FEV1 did not change significantly between the treatment and placebo group 26 .…”

“…Another RCT evaluated the effect of 5 mg desloratadine on a group of 20 patients with AR and exercise-induced bronchoconstriction where patients underwent an exercise challenge test before and seven days after receiving desloratadine or placebo 26 . “…Con respecto a la suspensión de antihistamínicos, también se deja a criterio del médico tratante. Se sugiere suspender 72 horas antes del examen, sin embargo se ha visto que loratadina, cetirizina, fexofenadina no afectan los resultados del estudio (12)(13)(14). El consumo de cafeína disminuye la reactividad bronquial por ejercicio por lo cual el paciente no debe ingerir este producto al menos 24 horas previo al examen.…”
