Introduction: Fertilization in humans is dependent on viability of the male spermatozoa among other factors and there have been conflicting reports on the role of pH, calcium and phosphate concentrations in sperm function. This study therefore aimed to investigate seminal plasma pH, inorganic phosphate, total and ionized calcium concentrations relative to spermatozoa function. Material and Methods: Seminal plasma concentrations of pH, total calcium, ionized calcium (Ca ++ ); inorganic phosphate, motility and spermatozoa count were determined in 80 males by standard methods.Results: Forty-nine of the subjects had normal spermatozoa motility (> 60%) and 31 had hypomotility (< 60%). The hypomotility group exhibited lower calcium ion (Ca 2 +) concentrations; 0.19+0.01mmol/L compared with normal motility group; 0.24+0.01mmol/L (p<0.001) the latter also had significantly higher inorganic phosphate; 7.83+1.27 while the former had 5.64+1.62mmol/L (p= 0.004). The mean spermatozoa counts for hypomotility and normal motility group were 42.0 ± 13 x 106 , 72.35 + 20 x 106 respectively (p< 0.001). No significant differences were observed in pH, volume of ejaculate and total calcium concentration between the hypomotility and normal motility groups The mean concentrations of pH were 7.51 ± 0.02 and 7.54 ± 0.03 respectively (p= 0.21) and total calcium; 3.10 ± 0.12 and 3.36 ± 0.14mmol/L respectively (p= 0.16 ). There was a significant difference in percentage of abnormal forms in both groups with hypomotile group having 36% compared to mormal motility group with 5% (p< 0.05). Conclusion:Correlations were observed between seminal concentrations of calcium ions, inorganic phosphate, spermatozoa count and motility but not with total calcium concentrations and pH and therefore should be considered in understanding male infertility and preparation of media for sperm preservation for in vitro fertilization.

A detailed sperm motility study for 24 h after collection was done. The level of calcium and magnesium in seminal plasma during this period was also seen to understand the role of these electrolytes on sperm motility. Good care was taken in selection of subjects (young and healthy), collection and pre-physical analysis of sample. Subjects (healthy, 18-25 years) were to maintain abstinence (2-5 days). Collection of sample into wide mouthed container provided was done next to laboratory by masturbation at 8 am ± 30 m. The total study was done in one month period. Samples were studied at ½, 1, 2, 4, 8, 12 and 24 h time for percentage of (1) total sperm motility, (2) active and progressive and (3) non-progressive motility and seminal plasma calcium and magnesium. Total sperm motility deteriorated to 50% in 8 h whereas active and progressive type reduced to 50% in 4 h. At 24 h total sperm motility was 9.8% and active and progressive motility 3.0%. Level of calcium (27.2 mg/dl) and magnesium (13.54 mg/dl) in seminal plasma did not show any significant changes during study period from that of at ½ h. The study concluded that electrolytes under study were not responsible for the decrease in motility during study period.

Severe oligoasthenoteratozoospermia (OAT) refers to impaired count, motility and abnormal sperm morphology of infertile men associated with high chromosomal abnormalities. The objective of the present study was to define a management protocol for severe OAT cases and discover new routes to improve their basic semen parameters. We have applied a therapeutic treatment protocol in a cohort of 210 infertile men diagnosed with extreme severe idiopathic OAT. This therapeutic treatment based on modifying the lifestyle factors combined with antioxidant treatment for 6 months in severe OAT to study its effect on basic semen parameter. Basic semen parameters were assessed before and after applying the therapeutic treatment strategy. Sperm concentration, percentage of total motility and progressive motility were significantly increased after applying the therapeutic treatment (p = .006, p = .001 and p = .001 respectively). On the other hand, abnormal sperm morphology was significantly reduced after therapy (p < .01). In conclusion, the present results suggested that antioxidative supplement in combination with modifying the lifestyle factors in a cumulative treatment period significantly improves the basic semen parameters.
