Effect of cirrhosis on antibiotic efficacy in a rat model of pneumococcal pneumonia

Abstract: A rat model was used to study the effects of cirrhosis on antibiotic therapy of pneumococcal pneumonia. Cirrhotic and control male Sprague-Dawley rats were infected transtracheally with type 3 Streptococcus pneumoniae. Treatment began 18 h later with phosphatebuffered saline (PBS), azithromycin (50 mg/kg), trovafloxacin (50 mg/kg), or ceftriaxone (100 mg/kg) injected subcutaneously twice daily for 5 days. Antibiotic concentrations were measured by high-performance liquid chromatography. Azithromycin, trovaflox… Show more

“…The impact of EtOH on the efficacies of antibiotics that have significant hepatic elimination is unknown. Trovafloxacin was as effective as ceftriaxone or azithromycin in a rat cirrhosis model of pneumococcal infection, but treatment with this antibiotic resulted in a significantly slower normalization of peripheral white blood cell counts (45). In the current study, we chose two different dosage regimens designed to achieve pharmacodynamic ratios for the lower dose just above the reported therapeutic threshold of clinical response.…”

“…We have previously demonstrated in an experimental model of hepatic cirrhosis that trovafloxacin has excellent efficacy, with superior area under the serum concentration-time curve (AUC)/MIC and maxi-mum concentration of drug in serum (C max )/MIC pharmacodynamic ratios compared to those of either a macrolide or a beta-lactam antibiotic (45).…”

“…In both cases, rats showed increased susceptibility to pneumococcal pneumonia. These models have been used by the same research group in studies on phagocytosis and antibiotic efficacy (84,184,199). A sophisticated surgical method based on the delivery of bacteria into the apical lobe bronchi of adult rats was described in the early 1990s (79).…”

“…12,31,32,33 Data from control, sham and LPT treatment rats were analyzed by analysis of variance (ANOVA) followed by a Tukey multiple comparisons post test using Graphpad Prism version 5.0 for Windows, (GraphPad Software, San Diego, CA). Differences among mean values with P £ 0.05 were considered statistically significant.Data are presented as arithmetic means ± standard error (SE).…”
