The levels of selected cariogenic and obligately anaerobic bacteria have been compared in carious dentine taken from fissures, smooth surfaces, and root surfaces. The numbers of infected dentinal tubules were determined by immunohistological staining using species-specific antisera against selected bacteria. Selective localization was observed; mutans streptococci were the predominant bacteria in dentine from fissures and smooth surface coronal caries, but not from root surface caries. The proportion of mutans streptococci was higher in the shallow and middle layers of dentine from fissures and smooth surface coronal carious lesions than from the deep layers. In root surface caries, Actinomyces spp. were the predominant bacteria, and were present at higher levels in the deep layer of root lesions than in the shallow and middle layers. The proportion of Lactobacillus spp. was relatively low despite its high detection frequency in all layers of the three types of carious lesion. Immunohistological staining with species-specific antisera was able to reveal the distribution and the localization of various bacteria in carious dentine.

Streptococcus mutans Out-competes Streptococcus gordonii in vivo

Streptococcus gordonii and Streptococcus mutans avidly colonize teeth. S. gordonii glucosyltransferase (GtfG) and amylase-binding proteins (AbpA/AbpB), and S. mutans glucosyltransferase (GtfB), affect their respective oral colonization abilities. We investigated their interrelationships and caries association in a rat model of human caries, examining the sequence of colonization and non- vs. high-sucrose diets, the latter being associated with aggressive decay in humans and rats. Virulence-characterized wild-types of both species and well-defined mutants of S. gordonii with interrupted abpA and gtfG genes were studied. While both S. gordonii and S. mutans were abundant colonizers of rat’s teeth in the presence of either diet, if inoculated singly, S. mutans always out-competed S. gordonii on the teeth, independent of diet, strain of S. mutans, simultaneous or sequential inoculation, or presence/absence of mutations of S. gordonii’s abpA and gtfG genes known to negatively or positively affect its colonization and to interact in vitro with S. mutans GtfB. S. mutans out-competed S. gordonii in in vivo plaque biofilm. Caries induction reflected S. mutans or S. gordonii colonization abundance: the former highly cariogenic, the latter not. S. gordonii does not appear to be a good candidate for replacement therapy. These results are consistent with human data.

The effect of multiple treatments with chlorhexidine mouth guards on salivary levels of mutans streptococci (MS) was studied. Eleven subjects with salivary MS ≥ 10⁵ CFU/ml were randomly assigned to an experimental or control group. Complete-arch mouth guards were coated internally with one of two ethylcellulose varnishes, one containing 30% dry w/w chlorhexidine and one containing no chlorhexidine. Subjects wore the mouth guards up to four times, each time for 7 consecutive nights while sleeping, with at least a 1-week break between treatment weeks. Subjects in the control group subsequently crossed over and also wore the chlorhexidine-containing mouth guards. Each of the week-long mouth guard sessions resulted in a reduction in mean salivary MS levels as follows (log₁₀ CFU/ml): for the experimental group, ––1.0, ––0.9, ––1.0 and ––1.2, and for the crossover group, ––1.7, ––0.6, ––0.5 and ––0.6. Salivary MS levels were (mean ± SD) 6.4 ± 0.5 at baseline and 3.8 ± 0.5 after the last mouth guard session for the experimental group and 6.0 ± 0.5 and 4.3 ± 0.7, respectively, for the crossover group. For the control group, there was no significant change in MS levels.
