Atopic dermatitis (AD) is a complex disease traditionally involving interaction of genetic, environmental, and immunologic factors. Recent studies suggest psycho-neuro-immunologic factors and emotional stress are important in its evolution. The observations that internal (bacterial infections) or external (psycho-logic) stressors may induce AD flares is explained by studies showing that stress impairs the skin barrier function and favors a shift in immunity toward a T helper type 2 cell/allergic response. Furthermore, those with AD appear to have an inherited hypothalamic deficiency that impairs normal hypothalamic-pituitary-adrenal axis function. Neuropeptides released in the skin may also mediate neurogenic inflammation, including mast cell degranulation. AD causes significant stress and impaired quality of life in patients and their family members. Psychologic and stress-reduction interventions were recently shown to improve patient well-being, and to significantly improve cutaneous manifestations.

Atopic dermatitis is a common inflammatory skin condition characterized by relapsing eczematous lesions in a typical distribution. It can be frustrating for pediatric patients, parents, and health care providers alike. The pediatrician will treat the majority of children with atopic dermatitis as many patients will not have access to a pediatric medical subspecialist, such as a pediatric dermatologist or pediatric allergist. This report provides up-to-date information regarding the disease and its impact, pathogenesis, treatment options, and potential complications. The goal of this report is to assist pediatricians with accurate and useful information that will improve the care of patients with atopic dermatitis. Pediatrics 2014;134:e1735-e1744Atopic dermatitis (AD), commonly referred to as eczema, is a chronic, relapsing, and often intensely pruritic inflammatory disorder of the skin. A recent epidemiologic study using national data suggested that the pediatric prevalence is at least 10% in most of the United States. 1 AD primarily affects children, and disease onset occurs before the ages of 1 and 5 years in 65% and 85% of affected children, respectively. 1 The number of office visits for children with AD is increasing. 2 Up to 80% of children with AD are diagnosed and managed by primary care providers, often pediatricians. 3 Although medical subspecialists, such as pediatric dermatologists and/or pediatric allergists, may be suited to provide more advanced care for children with AD, lack of a sufficient number of such physicians, particularly pediatric dermatologists, 4 likely means the burden of AD care will continue to fall to primary care providers. Although consensus guidelines and practice parameters regarding the management of AD in children have been published, 5-10 considerable variability persists in clinical practice, particularly regarding the roles that bathing, moisturizing, topical medications, and allergies play in management. Inconsistencies in opinion and treatment approach as well as the chronic and relapsing nature of AD can lead to frustration for the patient, family, and primary care providers when managing AD. STATEMENT OF THE PROBLEMNew data support the theory that AD results from primary abnormalities of the skin barrier, 11 suggesting that skin-directed management of AD is of paramount importance. This clinical report reviews AD and provides an up-to-date approach to skin-directed management that is based on pathogenesis. Effectively using this information to create treatment plans

Background: Chronic physical conditions in children significantly affect the quality of life in parents, especially mother. Many people with chronic pulmonary conditions as well as their family members need psychiatric assistance. Objectives: The goal of the current study was to evaluate the mental status as well as substance abuse behavior of mothers of asthmatic children. Patients and Methods:This was an observational cross-sectional study performed through completion of a questionnaire by participants. All mothers of asthmatic children at Masih Daneshvari Hospital from February 2011 to March 2013, consisted of 182 individuals, were included in the study after obtaining informed consents. The SCL-90 (symptom check list) questionnaire was completed by patients. History of substance abuse or cigarette smoking was asked from the sample group. Correlations between variables were analyzed using student t-test and chi-square test by SPSS-16 software (the statistical package for social science). Results:The mean age of participants was 33 ± 5 years and the average number of their children was 2 ± 1. The percentages of participants with scores in the abnormal range (≥ 2.5) were 17% for anxiety, 2% for obsessive compulsion, 15% for aggression, and 15% for the reference idea. Smoking had significant correlation with the presence of physical complaints (P < 0.007). Smoking history correlated with obsessivecompulsive behavior (P < 0.007) and substance abuse correlated with physical complaints (P = 0.001). Presence of physical complaints correlated with allergy in the children (P = 0.035). The total questionnaires' score and scores for each section also correlated (P = 0.000). Conclusions: This study showed that mothers of asthmatic children are prone to psychiatric symptoms. These symptoms included high scores on anxiety, aggression, reference idea and obsessive-compulsive thoughts. Scores on the physical complaint section were significantly higher in mothers whose children had allergic symptoms. Therefore, it is recommended that they get evaluated for psychiatric symptoms, and psychiatric treatment is recommended alongside treating asthma in their children.

Abstract: Compared with looking after a child with chronic asthma, caring for a child with chronic atopic eczema was associated with greater parental sleep disturbances. Disruption to parental sleep correlated with anxiety levels and, in the case of mothers, depression scores.
