Effect of balloon angioplasty in femoropopliteal arteries assessed by intravascular ultrasound

Abstract: Vascular damage is common following PTA. Lumen gain is mainly due to vessel expansion and, to a lesser extent, to a decrease in plaque area.

“…Both the lumen area and the vessel area at the target site increased significantly more than the mean values obtained from all cross-sections. In contrast, a slight increase in plaque area was seen, probably caused by a better detection of plaque after PTA; these findings correspond with studies in both coronary and peripheral arteries [3,5,6,11,13,14] (Table 4). However, in two other studies a different explanation has been proposed for the increase in lumen area after PTA.…”

“…13,14 In the FemPac study, 15 the rate of dissections was lower (~30% for both coated and uncoated balloons). In THUNDER, dissections occurred in~50% of the patients in each of the two treatment groups.…”

“…The main drawback of balloon angioplasty is vessel dissection, which occurs in ~80% of cases. 11,12 Severe dissection requires repeat revascularization and has an adverse effect on long-term patency. 10 Nitinol stent implantation is an alternative to balloon angioplasty, and midterm clinical outcomes of nitinol stenting were superior to those of balloon angioplasty for the treatment of femoropopliteal lesions.…”
