Effect of Atrial Fibrillation on Atrial Refractoriness in Humans

Abstract: In humans, several minutes of induced AF is sufficient to shorten the ERP for up to approximately 8 minutes. The temporal recovery of the ERP is reflected in progressively shorter episodes of reinduced AF. These data imply that AF transiently shortens the atrial wavelength and suggest a mechanism by which AF may perpetuate itself.

“…The study results did not, however, confirm the results of earlier studies, in animals as well as in humans [10][11][12], that patients with short duration of AF are more likely to remain in sinus rhythm after successful cardioversion. This might be explained by the fact that the duration of AF was long even in the patients with the shortest duration (e.g.…”

“…Time from start of AF is considered critical for success of cardioversion and for maintenance of sinus rhythm [10][11][12][13]. Although warfarin sodium treatment before cardioversion is recommended for 3-4 weeks at a therapeutic international normalized ratio (INR), it is not clear, in clinical practice, how long the actual time period is from start of warfarin sodium treatment to cardioversion.…”

“…Nevertheless, in our study the ERP measurements at different sites where not different between groups. Previous studies have demonstrated that AF lead to a decrease in atrial ERP, without a significant change in conduction velocity [18,19]. A shorter ERP can create a shorter wavelength (ERP × conduction velocity), which significantly contributes to the maintenance of AF [20].…”
