Effect of an extension tube on the bronchodilator efficacy of terbutaline delivered from a metered dose inhaler.

Abstract: A double-blind within-patient investigation was performed to determine whether the interposition of an extension tube (10 cm length x 3 -2 cm diameter) between a metered dose inhaler and the mouth alters the bronchodilator efficacy of terbutaline sulphate. On two consecutive study days 14 adult patients with stable reversible airways obstruction inhaled a cumulative dose of 500 jig of terbutaline which was delivered from a metered dose inhaler with or without the extension tube attached and received placebo in… Show more

“…The use of a spacer between the inhaler and the patient's mouth can potentially reduce the deposition of the drug on the oropharynx and may be preferred for patients who have difficulty using the inhaler correctly by the conventional method. [18][19][20] Another alternative is the "open mouth" method of administering inhaler medications, although evaluation of the efficiency of this method in drug delivery is still lacking. 21…”

“…Respiratory function parame ters selectively reflecting small airway di ameters were not included in their study. Both Gotnm et al [17] and Poppius [18] failed to find a significant effect of the ex tended mouthpiece. Gomtn et al [17] sug gest that patient selection may be a critical point.…”

“…2 This probably increases the potential for vasoactivity of the peripheral part of the vascular bed. 3 In infancy and childhood, abnormal vascular lung growth, associated with chronic hypoxia, was described. In victims of sudden infant death syndrome, increased muscularity of the pulmonary circulation was observed as evidenced by the extension of muscle into arteries not usually muscularized during the first year of life and by an increased thickness of the medial wall.…”
