Effect of Air Exposure and Suction on Blood Cell Activation and Hemolysis in an In Vitro Cardiotomy Suction Model

Abstract: Cardiopulmonary bypass (CPB) elicits a systemic inflammatory response. The cause may include surface-induced leucocyte activation and hemolysis. An in-vitro study was designed to describe the effects of both suction and an air-blood interface independently, and in combination on leucocyte and platelet activation, and hemolysis in an in-vitro model. Fresh human blood was drawn and tested under 4 different conditions including: control (A), 10 minutes of − 600 mmHg suction (B), 10 minutes of blood exposure to ro… Show more

“…An increase in CD11b expression on circulating monocytes indicates a response to an early inflammatory event [3, 19, 20]. The CD11b expression on rabbit monocytes was observed to significantly increase 1.6-fold after 4 h blood exposure to the NORel alone polymer ECC compared to no ECC baseline, but all combined argatroban/HMDI or PEG-DI/NORel or non-combined polymer ECCs exhibited no change or significant decreases in the CD11b expression from the no ECC baseline situation (Figure 11A).…”

“…Furthermore, the association of cystatin C with inflammation has been shown in other studies performed in non‐surgical cohorts . The systemic inflammatory reaction induced when blood comes in contact with non‐endothelialized surfaces in the CPB circuit is further enhanced if mediastinal and pericardial shed blood comes in contact with air when the blood returns into the CPB circuit . The inflammatory reaction is caused by activated leukocytes and an increased level of their activated mediators.…”

“…The test materials are exposed to blood in a closed syringe, which is commercially available, sterile and in medical grade quality. The design of the syringe allows the exclusion of air, which can artificially promote systemic blood activation due to the denaturation of plasma proteins, hemolysis, complement and platelet activation/aggregation during the test procedure [10,32]. In this study, a further modification of the blood-contacting surface -e.g.…”
