Effect of Age on Prognosis in Patients With Gastric Cancer

Abstract: Age clinically serves as a simple predictor of survival in patients with gastric cancer and should be taken into account along with conventional clinicopathological variables such as depth of invasion and lymph node metastasis.

“…Regarding lymph node involvement, if several reports did not show any significant difference in the rate of lymph node involvement (10,(28)(29)(30), other authors described a trend toward higher incidence of lymph node involvement in the elderly, although statistical significance was not reached (8,31); conversely in cases of early disease, several studies report lower prevalence of lymph node metastases in the elderly as compared to patients aged under 75 (7,27).…”

“…Peritoneum invasion emerges infrequently [29][30][32][33]. This distinct pattern of metastases and recurrence observed in the elderly may be related to the higher prevalence of blood vessels invasion observed in that age group [1,30]. Regarding the incidence of lymph node metastases, observations are controversial.…”

“…Regarding the incidence of lymph node metastases, observations are controversial. Several reports have revealed no significant difference in the rate of lymph node involvement [1,15,20,30], whereas few studies have demonstrated a trend toward higher incidence of lymph node metastases in the elderly, but not to a statistically significant extent [34]. In cases of early gastric cancer, numerous reports suggest a lower prevalence of lymph node metastases in the elderly as compared to younger patients (aged under 75) [14,16,35].…”

“…Some researchers thought that it was not an independent prognostic factor [38][39][40], whereas others thought that younger patients has worse prognoses than elderly due to the worse biological behaviors of tumors and histological type [41]. However, Saito et al held that elderly patients had worse prognosis because they had limited lymph node dissection and lower tolerance of chemotherapy [42].…”
