2) determine whether a second generation device was i) superior for reducing urge to smoke and withdrawal symptoms (WS) and ii) associated with enhanced positive subjective effects. Design:Mixed effects experimental design. Phase 1: reason for e-cigarette choice was assessed via questionnaire. Phase 2: participants were randomly allocated to first or second generation e-cigarette condition. Urge to smoke and WS were measured before, and 10mins after, taking 10 e-cigarette accounted for choice. Only baseline urge to smoke/WS predicted urge to smoke/WS 10 minutes after use (B=0.38; P<0.001 and B=0.53; P<0.001). E-cigarette device was not a significant predictor. Those using the second generation device were more likely to report satisfaction and use in a quit attempt (χ 2 =12.10, P=0.001 and χ 2 =5.53, P=0.02). Conclusions: First and second generation e-cigarettes appear to be similarly effective in reducing urge to smoke and WS during abstinence but second generation devices appear to be more satisfying to users.2

Once established, smoking is a very difficult addiction to break. Many smokers persist in tobacco use for several years and typically cycle through multiple periods of remission and relapse. Smoking cessation is not a single event but a process, and relapse is an ordinary component of this process. While international guidelines place great emphasis on relapse prevention, very little can be found about managing smokers who have relapsed. This article is intended to address the challenge of managing smokers who relapse in the course of a smoking cessation program. This knowledge may lead to an improved smoking cessation outcomes.

Current treatments for smoking cessation such as nicotine replacement therapy or varenicline address the primary reinforcer of smoking (nicotine), but sensorimotor stimuli (e.g. smell/taste of smoke, inhaling/exhaling, airway sensations, holding the cigarette) may act as secondary reinforcers and also contribute to smoking reward. Addressing both these aspects of smoking may help to enhance smoking cessation treatment. The aim of this review was to examine whether sensorimotor replacement can help to alleviate craving and aid smoking cessation. Three sensorimotor replacement products were examined: non-nicotine inhalators/aerosols, de-nicotinised cigarettes and electronic cigarettes. The current research suggests that sensorimotor replacement may enhance the efficacy of nicotine replacement therapy, but is unlikely to be useful if used alone. Electronic cigarettes may be the most promising approach, due to the combination of nicotine delivery and sensorimotor input.
