RESUMOObjetivos: Traduzir para o português, adaptar culturalmente e avaliar as propriedades psicomé-tricas do Summary of Diabetes Self-Care Activities Questionnaire − SDSCA. Materiais e méto-dos: O processo seguiu as normas internacionais para adaptação e avaliação das propriedades psicométricas. O alfa de Cronbach (a) foi utilizado para avaliar a consistência interna (correlação interitens) e a confiabilidade (teste-reteste e correlação interavaliador). Resultados: A correlação interitens variou de a = 0,09 a a = 0,86. Na avaliação teste-reteste, a menor correlação foi para o item "ingerir doces" (a = 0,15) e a maior, para os itens sobre o uso do cigarro (a = 1,00). As correlações interavaliador variaram de a = 0,29 a a = 1,00. Conclusões: O questionário adaptado apresentou propriedades psicométricas semelhantes às do SDSCA. Sua versão para o Brasil fornece um questionário confiável e válido para avaliar a aderência ao autocuidado nos diabéticos em nosso meio. Materials and methods: The process followed the international guidelines for the adaptation and evaluation of psychometric properties. The Cronbach's alpha (a) was determined to evaluate the internal consistency (inter-itens correlation) and the reliability (test-retest and interevaluator correlation). Results: The inter-itens correlation showed values of a = 0.09 to a = 0.86. In the test-retest evaluation, the lowest correlation was obtained for the item "eat sweets" (a = 0.15) and the highest correlation was obtained for the items concerning smoking (a = 1.00). The inter-evaluator correlations varied from a = 0.29 to a = 1.00. Conclusions: The adapted questionnaire showed psychometric properties similar to those of the SDSCA. Its Brazilian version provides a reliable and valid questionnaire to evaluate diabetic patient adherence to self-care in our community. Effectiveness and Safety of Patient Activation Interventions for Adults with Type 2 Diabetes: Systematic Review, Meta-Analysis, and Meta-regression

BACKGROUND: Patient activation interventions (PAIs) engage patients in care by promoting increased knowledge, confidence, and/or skills for disease self-management. However, little is known about the impact of these interventions on a wide range of outcomes for adults with type 2 diabetes (DM2), or which of these interventions, if any, have the greatest impact on glycemic control. METHODS: Electronic databases were searched from inception through November 2011. Of 16,290 citations, two independent reviewers identified 138 randomized trials comparing PAIs to usual care/control groups in adults with DM2 that reported intermediate or longterm outcomes or harms. For meta-analyses of continuous outcomes, we used a random-effects model to derive pooled weighted mean differences (WMD). For allcause mortality, we calculated the pooled odds ratio (OR) using Peto's method. We assessed statistical heterogeneity using the I 2 statistic and conducted meta-regression using a random-effects model when I 2 >50 %. A priori meta-regression primary variables included: intervention strategies, intervention leader, baseline outcome value, quality, and study duration. RESULTS: PAIs modestly reduced intermediate outcomes [A1c: WMD 0.37 %, CI 0.28-0.45 %, I 2 83 %; SBP: WMD 2.2 mmHg, CI 1.0-3.5 mmHg, I 2 72 %; body weight: WMD 2.3 lbs, CI 1.3-3.2 lbs, I 2 64 %; and LDLc: WMD 4.2 mg/dL, CI 1.5-6.9 mg/dL, I 2 64 %]. The evidence was moderate for A1c, low/very low for other intermediate outcomes, low for long-term mortality and very low for complications. Interventions had no effect on hypoglycemia (evidence: low) or short-term mortality (evidence: moderate). Higher baseline A1c, pharmacistled interventions, and longer follow-up were associated with larger A1c improvements. No intervention strategy outperformed any other in adjusted meta-regression. CONCLUSIONS: PAIs modestly improve A1c in adults with DM2 without increasing short-term mortality. These results support integration of these interventions into primary care for adults with uncontrolled glycemia, and provide evidence to insurers who do not yet cover these programs. Purpose: To identify factors related to changes in foot status and foot care in patients with diabetes mellitus participating in an exercise program. Methods: The subjects were 21 patients with Type 2 diabetes mellitus (8 males, 13 females). The mean age of the patients was 62.3 ± 12.5 years old. Foot status, physical measurements and daily activities were monitored and several surveys were performed. Results: Changes in foot status were significantly positively correlated with walking at normal speed (r = 0.54, p < 0.05) and fast speed (r = 0.68, p < 0.01), and significantly negatively correlated with foot care (r = −0.51, p < 0.05). Foot care showed a significant negative correlation with burden of diet therapy (r = −0.48, p < 0.05) and a significant positive correlation with the self-efficacy of diet therapy (r = 0.65, p < 0.01). Conclusions: Subjects who were fast walkers showed greater changes in foot status. Poor foot care behavior is also related to greater changes in foot status. Regular interventions for foot care are important to improve self-care behavior during an exercise program.
