(1) Objective. We aimed to demonstrate that the use of the ultrasound-guided technique facilitates peripheral venous cannulation as compared to the standard technique in patients with difficult access at emergency services. (2) Method. A case–control study, randomized research. Variables were collected from a population with non-palpable or not visible veins, classified into size risk groups for 6 months. In the comparative analysis, the patients were divided into two groups: the cases group was composed of patients to whom the peripheral venous cannulation was performed with the ultrasound-guided technique (UST), while the control was composed of patients with whom the standard technique (ST) was performed. The ultrasound LOGIQ P5 750VA from General Electric Healthcare, with an 11 mHz linear probe, was utilized, along with peripheral venous catheters model InsyteTM AutoguardTM with gauges of 14G to 26G. (3) Results. Seventy-two cases. The use of the ultrasound decreased the time (618.34s ST, 126s UST) and the number of punctures (2.92 ST, 1.23 UST); about 25% of the patients did not have complications with the UST, as compared to 8% with the ST. The use of the ultrasound decreased the pain experienced by 1.44 points in the visual analog scale, as compared to 0.11 points with the ST. The rate of success of the first try with the UST was 76%, as compared to 16% of the ST. The gauge of the catheter increased with the UST, with successful cannulations obtained with 20G (56%) and 18G (41%) gauges. (4) Conclusions. The use of ultrasound facilitates venous cannulation according to the variables of the study. The ultrasound visualization of the vessels is associated with the selection of the catheter gauge. There was no relation between the complications and the depth of the blood vessels.

Identificar os motivos que norteiam a tomada de decisão para usar ou não a ultrassonografia na punção periférica na prática clínica de enfermagem. Trata-se de uma revisão integrativa de artigos publicados na íntegra, em periódicos indexados em bases nacionais e internacionais. Foram identificados 2.247 artigos segundo os descritores selecionados dos quais 36 atenderam os critérios de inclusão e exclusão, segundo três categorias: tecnologia, experiência do paciente e capacitação para o uso. Tanto a necessidade, quanto a importância do uso da ultrassonografia a foram evidenciados nos estudos, porém, lacunas para embasar a decisão do enfermeiro para usar ou não esta tecnologia na prática clínica foram observadas nos estudos. A ultrassonografia tem sido usada na punção venosa periférica, contribuindo para a segurança do paciente, no entanto é necessário ferramentas de decisão para apoiar o profissional quanto ao uso da tecnologia. Decision making for the use of ultrasound for difficult peripheral venipuncture in adults: an integrative review Abstract: To identify the fundamentals that guide decision making to use or not to use ultrasound in peripheral puncture in clinical nursing practice. This is an integrative review of articles published in full, in journals indexed on national and international bases. 2,247 articles were identified according to the selected descriptors and among these, 36 met the inclusion and exclusion criteria, according to three categories: technology, patient experience and training for use. Both the need and the importance of using ultrasonography were highlighted in the studies, however, gaps to support the nurse's decision to use this technology or not in clinical practice were observed in the studies. Ultrasonography has been used in peripheral venipuncture, contributing to patient safety, however, decision tools are needed to support the professional regarding the use of technology. Toma de decisiones para el uso de la ecografía en la venopunción periférica difícil en adultos: una revisión integradora Resumen: Identificar los fundamentos que orientan la toma de decisiones sobre el uso o no de la ecografía en la punción periférica en la práctica clínica de enfermería. Se trata de una revisión integradora de artículos publicados íntegramente, en revistas indexadas a nivel nacional e internacional. Se identificaron 2.247 artículos según los descriptores seleccionados y de ellos, 36 cumplieron con los criterios de inclusión y exclusión, según tres categorías: tecnología, experiencia del paciente y formación para su uso. Tanto la necesidad como la importancia del uso de la ecografía se destacaron en los estudios, sin embargo, en los estudios se observaron lagunas para sustentar la decisión de la enfermera de utilizar o no esta tecnología en la práctica clínica. La ecografía se ha utilizado en la punción venosa periférica, contribuyendo a la seguridad del paciente, sin embargo, se necesitan herramientas de decisión para apoyar al profesional en el uso de la tecnología.
