To evaluate the analgesic effect of esmolol in patients submitted to laparoscopic gastroplasty. Methods: Forty patients aged between 18 and 50 years with American Society of Anesthesiologists (ASA) physical status scores of II and III who underwent gastric bypass were allocated to two groups. Group 1 patients received a 0.5-mg/kg bolus of esmolol in 30 mL of saline before induction of anesthesia, followed by an infusion at 15 µg/kg/min until the end of surgery. Group 2 patients received 30 mL of saline as a bolus and then an infusion of saline. Anesthesia included fentanyl (3 µg/kg), propofol (2-4 mg/kg), rocuronium (0.6 mg/kg), and 2% sevoflurane, with remifentanil if necessary. The following parameters were evaluated: pain intensity over 24h, remifentanil consumption, the first analgesic request, morphine consumption, and side effects. Results: Pain intensity was lower in the esmolol group except at T0 (after extubation) and 12h postoperatively. Remifentanil supplementation, recovery time, and postoperative morphine supplementation were lower in the esmolol group. No differences in the time to the first analgesic request or side effects were found between the groups. Conclusion: Intraoperative esmolol promotes reductions in pain intensity and the need for analgesic supplementation without adverse effects, thus representing an effective drug for multimodal analgesia in gastroplasty.

O esmolol é um antagonista cardiosseletivo do receptor ß1 de ação ultracurta. É eficaz no embotamento das respostas aos estímulos adrenérgicos perioperatórios, incluindo intubação traqueal, eventos intraoperatórios causados pela diminuição da profundidade anestésica e extubação traqueal. Desta maneira, foi objetivo deste estudo analisar na literatura nacional e internacional os efeitos do esmolol no período periopertaório e pós-operatório de cirurgias não cardíacas. Trata-se de uma revisão integrativa de literatura realizada na Biblioteca Virtual da Saúde, sendo selecionados os artigos da base de dados da literatura científica e técnica da América Latina e Caribe (LILACS) e do Medical Literature Analysis and Retrieval System Online (MEDLINE). Os resultados revelaram que administração de esmolol trouxe benefícios, não sendo mencionado nenhum maléficio com uso, onde em infusão essa medicação é mais eficaz que em bolus para controlar da pressão arterial sistólica durante a intubação endotraqueal e esternotomia, reduzindo significativamente a necessidade de anestésico-analgésico, dor e incidência de NVP. As pesquisas também meniconaram que o esmolol teve um efeito poupador de opióides no período intraoperatório e pós-operatório imediato em pacientes hipertensos submetidos à laparoscopia. Esmolol em infusão intravenosa reduziu o consumo de analgésicos tanto no intraoperatório quanto no pós-operatório, reduziu os escores da escala analógica visual no pós-operatório imediato e prolongou o tempo para a primeira analgesia. Portanto, foi possível constatar que o esmolol é uma medicação segura, com muitas vantagens e com poucos efeitos adversos.

Background: Both intraoperative esmolol and transversus abdominis plane (TAP) block facilitate postoperative analgesia after laparoscopic cholecystectomy as part of multimodal analgesia. Both strategies can minimize the use of postoperative opioids. In current study, our goal was to assess if intra-operative esmolol infusion in association with TAP block can overcome the deficits of TAP block alone after laparoscopic cholecystectomy. Methods: This prospective, randomized and double-blinded clinical trial included 60 patients of either sex who scheduled for elective laparoscopic cholecystectomy; received either ultrasound-guided TAP block alone or in association with intravenous esmolol bolus (0.5 mg/kg) before induction followed by a maintenance infusion (0.05 mg/kg/min) till the end of operation. Intra-operative hemodynamic parameters were followed up. Postoperatively, in order to maintain visual analogue scale (VAS) scores ≤3, patients received IV morphine. The primary outcome was amount of opioid consumption during the first 24 hours postoperative. Pain scores, mean arterial pressure (MAP), heart rate (HR) and occurrence of nausea/vomiting were secondary outcomes. Results: The mean morphine consumption after surgery in patients receiving esmolol was (5.83) mg compared to (7.5) mg in TAP only group (p = 0.204). The mean pain scores at early postoperative hours were significantly lower in esmolol group (p < 0.05). MAP and HR were significantly lower in esmolol group intraoperative; however, no variance was detected later. Conclusion:In the first 24 hours following surgery, esmolol infusion increased the analgesic impact of TAP block in terms of opioid demand and pain severity.
