Efavirenz or nevirapine in three-drug combination therapy with two nucleoside or nucleotide-reverse transcriptase inhibitors for initial treatment of HIV infection in antiretroviral-naïve individuals

Abstract: BackgroundThe advent of highly active antiretroviral therapy (ART) has reduced the morbidity and mortality due to HIV infection. The World Health Organization (WHO) ART guidelines focus on three classes of antiretroviral drugs, namely nucleoside or nucleotide reverse transcriptase inhibitors (NRTI), non-nucleoside reverse transcriptase inhibitors (NNRTI) and protease inhibitors. Two of the most common medications given as first-line treatment are the NNRTIs, efavirenz (EFV) and nevirapine (NVP). It is unclear … Show more

“…The comparison of PI with NNRTI is a mixed comparison of NVP and EFV. However, previous studies have shown that NVP and EFV have similar benefits in initial treatment of HIV infection when combined with two NRTIs 22 .…”

“…Non-nucleoside reverse transcriptase inhibitors (NNRTIs) are the drugs most commonly implicated in hepatotoxicity. The two most commonly used NNRTIs Nevirapine and Efavirenz are frequently used as part of the triple combination first-line ART regimen [ 5 ]. Nevirapine use has been associated with significant hepatic injury compared to Efavirenz [ 6 ].…”

“…None of the side effects were severe enough to require discontinuation. However, clinical observed side effects, predominantly persistent neuropsychiatric symptoms were observed of virological success and progression to AIDS (10). Studies from India have shown good tolerability with both nevirapine and efavirenz (11)(12)(13)(14).…”
