Studies have shown the possibility of using the part of the foreskin removed after circumcision in the field of scientific and therapeutic research. Donations of tissues and organs are always associated with ethical challenges posed by bioethicists and societies to ensure the appropriate use of these tissues/organs. The purpose of this study was to understand the attitudes and awareness of parents/guardians regarding donation of excised foreskin to research and medical use. The study was based on a questionnaire and included 133 parents/guardians who visited Uhud Children’s Hospital in Madinah, Saudi Arabia for newborn male circumcision. The results showed a high willingness (61.7%) to donate the extracted foreskin to research. The willingness to donate the extracted foreskin to research associated with undergraduate degree (P = 0.018), male sex (P = 0.011), high income (P = 0.029), and participation in previous research studies (P = 0.002). About 41.8% were convinced that written informed consent should be obtained before circumcision surgery, 38.1% (n = 51) were convinced that written informed consent should be taken after surgery, while the remaining 19.4% reported that the timing of written informed consent is unimportant. Finally, fear of excision of excess tissue (74.5%), lack of confidence in the research (68.6%), and potential for commercial use (64.7%) were the main barriers to unwillingness to donate the excised foreskin for research. In conclusion, a reasonable portion of Saudis agreed to donate their foreskin for research purposes. There is an urgent need to enhance awareness and attitudes towards tissue donation for research and therapeutic use.

“…Human tissue banks (HTB) are public or private institutions specialized in health services and mainly responsible for capturing, processing, storing and distributing human tissues for transplants and research from living and/or deceased donors (autografts/ allografts) 1,2 . Among the many types of tissues that can be transplanted, the following stand out: skin, amniotic membrane, corneas, vessels, valves, bones, tendons, menisci, fascia, and cartilage, among others 2,3 The products made available by HTB are mainly intended to meet the clinical demands of patients on the waiting list managed by the National Transplant System (SNT), with repair, regeneration and/or tissue failure needs, with these tissues being exempt from the need for histocompatibility with the donor [3][4][5] . The Brazilian Ministry of Health’s Consolidation Ordinance No. 4 of September 28, 2017, provides the requisites for the evaluation of services for human tissue banks (bancos de tecidos humanos [BTH]) in terms of the microbiological control of environments (ISO 5 classification processing room) and products (culture for aerobic, anaerobic, and fungal pathogens), with a view to guaranteeing harmlessness in tissue transplants. Objectives: To highlight the techniques for microbiological control of distributed tissues adopted by a BTH over 5 years, as well as the results obtained from the application and use of these techniques. Methods: Using standard operating procedures (SOPs), the methodology, and frequency of collections, tests and room cleaning were established in advance. Microbiological cultures were taken from the tissues during all the collections and processing. During tissue processing, fingerprint samples on the processor’s gloves and in the processing room were seeded on blood agar plates. The processing room was cleaned weekly with biguanide and quaternary ammonium. The processing bench was sanitized with sterile 70% alcohol and the microbiological environmental control was carried out every 6 months by a qualified outsourced company. Results: In the period analyzed, the techniques proved effective in 46 (96%) cases, with contamination identified in only two (4%) processed and collected samples. The effectiveness and results were documented. Conclusion: Effective biological quality control methods are legitimately required, but they can be improved, raising the need to develop safe protocols for the quality of services and the safety of transplanted tissues.

Introdução: A negativa familiar para autorização da doação de órgãos e tecidos humanos mantém-se elevada ao longo dos anos. Dentre as muitas causas de não efetivação da doação, destacam-se a falta de informação sobre o processo e o desconhecimento da família sobre o desejo do doador em vida, além de outras questões, como a aparência do corpo do doador após a captação. Nesse sentido, torna-se necessário trazer à luz da discussão os aspectos que permeiam os conceitos de morte, dignidade humana e zelo pela imagem do doador, principalmente no caso de doadores de tecidos, nos quais a retirada pode apresentar-se mais perceptível. Objetivos: Realizou-se uma revisão integrativa da literatura (RI), buscando evidenciar lacunas e caracterizar o processo da doação de órgãos e tecidos humanos, com foco na reconstrução do corpo do doador e na dignidade humana. Em complemento, também se propôs descrever um relato de experiência de um profissional de um banco de tecidos humanos (BTH). Métodos: Por meio da estratégia PICO, elaborou-se a questão norteadora do estudo, após a RI realizada nos últimos 10 anos, operacionalizada por descritores controlados nas bases de dados SciELO, LILACS, Google Acadêmico e PubMed. Adicionado à busca, foi descrito um relato de experiência referente às técnicas de reconstrução do corpo do doador após a captação dos tecidos utilizados por um BTH localizado no interior do estado de São Paulo. Resultados: Foram encontrados nove estudos específicos sobre doação e utilização de próteses para reconstrução de áreas doadas. Os resultados demonstram os motivos que envolvem a recusa familiar, assim como o cuidado que as equipes técnicas têm na reconstituição do corpo dos doadores. A preocupação da família em relação à situação estética do doador influencia a aceitação para doação, portanto, torna-se necessário investir na seguridade e na qualidade dos serviços prestados pelos BTH, a fim de diminuir a recusa familiar para doação. Conclusão: A comunicação clara e a seguridade dos processos que envolvem a captação de órgãos e tecidos humanos mostraram-se o melhor caminho para facilitar a doação, podendo, assim, aumentar as doações.
