Характеристика сучасних методів діагностики уролітіазуВступ. Сечокам'яна хвороба (СКХ) є досить частим захворюванням. Тому проблема нефролітіазу набуває все більшої актуальності Важливість проблеми зумовлена ще й тим, що захворювання найчастіше зустрічається серед людей найбільш працездатного віку -від 20 до 50 років. Абсолютна кількість зареєстрованих хворих на сечокам'яну хворобу зростає щороку на 12-20%. Нирковокам'яна хвороба є однією з найчастіших причин хронічної ниркової недостатності (ХНН) у урологічних хворих Розвиток найважчого ускладнення -гострого калькульозного пієлон.ефриту -створює ургентну ситуацію.Мета роботи. Провести аналіз літературних джерел з метою аналізу ефективності застосування сучасних методів в діагностиці уролітіазу.Матеріали та методи. Здійснено науковий пошук за період з 2000 по 2022 рр. за первинною пошуковою стратегією «діагностика уролітіазу» (diagnosis of urolithiasis), «сечокам'яна хвороба» (urolithiasis), та комбінацією AND/OR в наукометричних базах MEDLINE (PubMed), Scopus, Web of Science.Результати дослідження. За наявності конкремента або розширення порожнинної системи нирки проводиться оглядова урографія; яка, поділяє конкременти нирок на дві групи, рентгеннегативні та рентгенпозитивні, що дозволяє побічно судити про хімічний склад конкременту. За результатами оглядової урографії та ульразвуковому дослідженні нирок на основі клініки та лабораторних даних можна припустити можливість застосування малоінвазивних методів видалення конкрементів. Достатньою підставою для відмови від рентгенологічного дослідження виявлення при ехографії конкрементів у чашково-лоханковій системі, а також ускладнень у вигляді гідрокалікозу або гідронефрозу. Інформативність екскреторної урографії обмежена також у хворих при нирковій коліці на висоті больового нападу, при нефункціонуючій нирці та хронічній нирковій недостатності.Висновки. Пріоритетними методами дослідження нефро-уролітіазу слід вважати радіологічні, де перевага віддається мультиспіральній комп'ютерній томографії, у решті випадків першою лінією діагностики є ультразвукова доплерометрія з картуванням. Функція нирки остаточно вирішується при виборі радіоізотопних рентгеносцинтіграфій.

Introduction. In recent years, there has been more and more research on the creation of artificial intelligence (AI) to help diagnose and treat patients in various fields of medicine. The use of algorithms allows us to reduce the time for processing large volumes of information and give a «second opinion» to a specialist in complex and non-standard clinical cases. In order to increase the effectiveness of percutaneous nephrolithotripsy, we have created an AI algorithm to personalize approach in the surgical treatment of nephrolithiasis. Purpose. To develop a universal tool for predicting the most optimal tactics of percutaneous nephrolithotripsy depending on the clinical case. Materials and methods. The total number of patients treated at N. Lopatkin Scientific Research Institute of Urology and Interventional Radiology and those who took part in the study amounted to 1000 people. The number of men was 419 (41.9%), women 581 (58.1%). The age of patients included in the studies ranged from 18 to 88 years (52.3±13.47). The division into Training and Holdout partitions varied depending on the target variable to obtain the most accurate result. The maximum difference in the samples was 600n and 400n (60% and 40%), the minimum 800n and 200n (80% and 20%). Data processing was carried out using IBM SPSS Statistics and Modeler programs using the neural network modeling method. Results. The accuracy of prediction for the choice of nephroscope size was 82.2%, requirement for intraoperative stent placement 93.9%, requirement for ureteroscopy or contact ureterolithotripsy 98.5%. The accuracy of the forecast for the number of puncture accesses was 92.6%, for access through the upper group 95%, for access through the middle group 91.2%, for access through the lower group 91.2%. The algorithm allows us to predict the presence of residual stones with an accuracy of 84.1%, the duration of surgical intervention is 87.3%.The number of test observations was 30. The algorithm demonstrated high accuracy of prediction (from 73.8% to 94.5%) of surgical tactics in complex clinical cases. Conclusion. The results of using the algorithm for personalized prognosis in the surgical treatment of nephrolithiasis showed high accuracy and efficiency. Further development of technology to create algorithms for each nosology and surgical interventions will reduce the time of data analysis by the attending physician, provide a «second opinion» to young specialists and standardize the approach to treatment in rare and complex clinical cases.

Purpose of review Simulation is a key component of surgical training, enabling trainees to develop their skills in a safe environment. With simulators broadly grouped into physical models and virtual-reality (VR) simulators, it is important to evaluate the comparative effectiveness of the simulator types in terms of validity as well as cost. The review aims to compare the benefits and drawbacks of novel VR and physical simulators within the broader themes of endourology, laparoscopic and robotic operations, and other urological procedures. Recent findings Key benefits of bench models include their comparatively lower cost, easy access and provision of haptic feedback, whereas VR simulators are generally self-sufficient, reusable and enable skills of haemostasis to be practised. The advent of perfused 3D printed simulators across a range of urological procedures may replace cadavers as the traditional gold-standard simulation modality. Summary Although possessing differing strengths and downsides, VR and physical simulators when used together can have an additive effect due to skill transferability across the platforms. Further comparative studies are required to directly quantify the differences between physical models and VR simulators in terms of performance metrics and cost-effectiveness. There is lack of validated VR simulators for open and reconstructive procedures.
