SummaryFacial edema is a relatively frequent clinical presentation encountered in patients seen in allergology and dermatology clinics. The differential diagnosis is broad, and sometimes the definitive diagnosis can be a challenge for the clinician. Facial angioedema itself encompasses different etiopathologies (histaminergic, bradykinergic, etc.) that must be distinguished from other causes of facial edema, such as allergic contact dermatitis, granulomatous conditions, inflammatory causes, infections, neoplasms or paraneoplastic syndromes, autoimmune diseases, among other entities hereby referred as miscellanea. A proper diagnostic approach is essential to order the appropriate tests, as well as to prescribe a targeted treatment. This review focuses on entities that present with facial edema and summarize their characteristic clinical features.

ZusammenfassungGesichtsödeme sind ein relativ häufiges Symptom bei Patienten allergologischer und dermatologischer Kliniken. Die Differentialdiagnose ist breit gefächert und die endgültige Diagnose kann manchmal eine Herausforderung für den Kliniker sein. Das faziale Angioödem selbst umfasst verschiedene Ätiopathologien (histaminerg, bradykinininduziert und weitere), die von anderen Ursachen des Gesichtsödems differenziert werden sollten, etwa allergischer Kontaktdermatitis, granulomatösen Erkrankungen, inflammatorischen Ursachen, Infektionen, Neoplasmen oder paraneoplastischen Syndromen, Autoimmunerkrankungen und anderen Entitäten, die hier als „Sonstige“ bezeichnet werden. Um die richtigen Untersuchungen anzuordnen und eine gezielte Therapie zu verordnen, ist ein angemessener diagnostischer Ansatz ausschlaggebend. Diese Übersicht konzentriert sich auf Entitäten, die mit Gesichtsödem einhergehen, und fasst ihre klinischen Charakteristika zusammen.

Linfoma MALT: un diagnóstico diferencial de edema periorbitario

El edema periorbitario es un signo clínico que presenta diversidad de causas, las cuales deben ser contempladas durante el abordaje, con la finalidad de iniciar el tratamiento lo más pronto posible. Algunas de estas etiologías pueden ser infecciosas, inflamatorias, medicamentosas o tumorales, como en este caso. El linfoma de células B extranodal de la zona marginal del tejido linfoide asociado a mucosas (linfoma MALT) es un subtipo de Linfoma No Hodgkin, cuya ubicación más común es a nivel gástrico. Se puede encontrar en otros tejidos como la órbita, habitualmente, en mujeres adultas, con manifestaciones como una masa acompañada de proptosis. El estudio determinante es la biopsia, aunque se requieren exámenes adicionales para el estadiaje. El tratamiento en caso de infiltración sistémica es quimioterapia, por lo general, con un buen pronóstico. En este artículo se presenta el caso de una paciente femenina de 66 años con historia de más de 2 años de evolución de edema periorbitario bilateral, asociado a síntomas B. En la biopsia palpebral se demostró linfoma MALT, posteriormente, se comprobó diseminación sistémica, incluida medular, por lo que se le asignó un estadio IVBE. Debido a esto, se inició protocolo R-CHOP, con adecuada respuesta.
