Ectopic Atrial Tachycardia in an Infant With Transient Erythroblastopenia of Childhood

Abstract: Transient erythroblastopenia of childhood is a self-limited anemia occurring in previously healthy children, secondary to temporary cessation of erythrocyte production. Although the precise etiology is unclear, most cases are associated with a viral illness. The anemia may be severe, with associated pallor, tachypnea, and tachycardia; treatment is supportive. We present an unusual case of a child with viral-induced transient erythroblastopenia of childhood and associated ectopic atrial tachycardia, requiring t… Show more

“…Considering, the main signs/symptoms and lab findings of our patient at presentation, such as skin and mucosal pallor, tachycardia, a cardiac flow murmur, reticulocytopenia, and a spontaneously resolving normocytic anemia, our patient was diagnosed with TEC. Altogether, this particular case was mostly consistent with TEC presentations seen in the literature [4].…”

“…More recently prospective analyses of similar cases have failed to localize a specific viral agent responsible for TEC [ 3 ]. However, they often present with typical anemic symptoms such as mucosal pallor, cardiac flow murmurs, and tachycardia [ 4 ]. Parents of the patients often mention reduced energy levels and elevated fatigue in children with this disease.…”

“…Low hemoglobin levels has been shown to be an important factor in increasing cardiac output to maintain adequate oxygen supply to the tissues (14) . When hemoglobin concentration decreases, body may increase cardiac output to maintain the normal metabolic demands of tissues, which increases cardiac work, and result in tachycardiaand may trigger AVNRT via atrial ectopic beats (15) .…”

“…Nasuprot tome, pacijenti s TEC-om urednog su nalaza osim znakova anemije 7 . Opisani su pojedinačni slučajevi TEC-a s ektopičnom atrijalnom tahikardijom, perikarditisom i neurološkim promjenama 2,9 . Najčešći infektivni uzročnik stečene aplazije crvene loze je parvovirus B19, a opisani su i virus hepatitisa A, B i C, Epstein-Barrov virus i virus humane imunodeficijencije 3,5,9 .…”

“…Često su jedini znak bolesti bljedoća kože i sluznica. Mogu biti prisutni drugi simptomi anemije: bezvoljnost, umor, srčani šumovi, tahikardija, tahipneja i najteža komplikacija hemodinamska nestabilost s potrebom za transfuzijskim liječenjem 1,2 . Etiologije nepoznata, a čest je podatak o preboljeloj virusnoj infekciji unatrag dva do tri mjeseca, najčešće parvovirusom B19 ili humanim herpes virusom 6 3 .…”
