Ectopic ACTH Secretion in a Child With Metastatic Ewing's Sarcoma: A Case Report

Abstract: Ectopic ACTH syndrome is rare in pediatric age. Sarcomas that cause Ectopic ACTH Syndrome (EAS) are even more uncommon. Currently, only three cases of EAS caused by Ewing' sarcoma have been reported. We detail a 10-year-old boy with Cushing's syndrome symptoms caused by ectopic ACTH production by a metastatic Ewing's sarcoma of the right ischio-pubic and ileo-pubic branches. The rapid appearance of cushingoid symptoms, with significant weight gain, acne, hirsutism, and hypercortisolism were implications of ect… Show more

“…The fourth case was of a ten-year-old boy from Italy with Cushing's syndrome caused by ectopic ACTH production from a metastatic ES tumor of the right ischio-pubic rami with multiple secondaries who was managed with chemotherapy but succumbed to the illness (9). The present case assumes significance in view of being the 1 st case of ES of the humerus with ectopic ACTH.…”

“…Ectopic ACTH caused by sarcomatous tumors is still rarer (4,5). As far as we were able to determine, only four patients with ACTH-dependent Cushing's syndrome caused by ES have been reported to date (6,7,8,9). We report the first case of ectopic ACTH with ES arising from the humerus.…”

“…On the other hand, there are those that believe its lineage arises from neural crest cells, which one would expect would make it more predisposed to neurologic paraneoplastic syndromes [7]. The few cases of paraneoplastic syndrome with Ewing sarcomas that have been reported have been endocrinopathies involving Cushing's syndrome and ectopic ACTh release [8][9][10]. This case is believed to be the first to have an association between Ewing sarcoma and Guillain-Barre Syndrome.…”

“…In the present study, we evaluated the expression of 6 important miRNAs (miR-21-5p, miR-199a-5p, miR155-5p, miR-34a-5p, miR-145-5p, and miR-218-5p) selected based on results of several previous studies complete database search 19 , 20 . Some of these miRNAs are also found to be associated with HR-HPV infection in cervical pre-cancer and cancer 21 , 22 .…”
