PurposeThe resource crisis in healthcare can be alleviated by engaging external stakeholders and resources in healthcare delivery. The authors use value and open innovation concepts to understand what motivates the stakeholders to join the healthcare innovation ecosystem and what value such an ecosystem brings to healthcare.Design/methodology/approachA systematic literature review following the PRISMA framework method was applied to reach the research objective. Out of a total of 509 identified articles published till 2021, 25 were selected as relevant for this review.FindingsSix categories of actors were identified, including innovation intermediaries, which were so far neglected in the healthcare innovation literature. Furthermore, patients, healthcare providers, innovation suppliers, investors and influencers were described. The authors also distinguished internal and external stakeholders. The authors show why and how open innovation projects contribute to involving external stakeholders and resources in healthcare delivery by contributing to patient autonomy, relationship building, knowledge transfer, improving collaborative mindset and culture, advancing know-how and bringing additional finances.Originality/valueThis article is the first one to systematically describe the value of open innovation in healthcare. The authors challenge the positivist approach in value presented by value-based healthcare. The authors show how openness contributes to addressing the resource crisis by involving new stakeholders and resources in the care delivery process.

The ageing of world population has a direct impact on the health and care systems, as it means an increase in the number of people needing care which leads to higher care costs and the need for more resources. In this context, informal caregivers play an important role as they enable dependent persons to stay at home and thus reduce care costs. However, long-term continuous care provision has also an impact in the physical and mental health of the caregivers. Moreover, geographical barriers make it difficult for caregivers to accessing psychoeducation as a way to alleviate their problems. To support caregivers in their needs and provide specialized training, technology plays a fundamental role. The present work provides the theoretical basis for the development of a technological ecosystem focused on learning and knowledge management processes to develop and enhance the caregiving competences of formal and informal caregivers, both at home and in care environments. In particular, a platform-specific model to support the definition of the ecosystem based on Open Source software components is presented, along with a Business Model Canvas to define the business structure as part of the human elements of the technological ecosystem.

Service science is concerned with the question of how systems can cocreate value in an optimal way. In essence, innovations aim at enabling better value co-creation; but at the same time, cause disruption and tensions in the service ecosystem by challenging prevailing practices. This chapter examines the development and diffusion of a broad scale heath care service innovation -the Electronic Prescription system (eRX) -as a process of institutionalization within a service ecosystem. This case represents an innovation process that attempts to solve a major societal challenge, rationalization of medication and reduction of medication errors and abuse. This change requires commitment and adaptation by diverse actors in multiple service systems affected by the eRX, but is nearly disabled by these actors' competing and even conflicting institutional logics. We examine how diverse stakeholders slowly move towards a convergent institutional logic as the innovation is gradually institutionalized in the broader service ecosystem, and discuss the major challenges along this process. This chapter highlights the dilemma of change in service ecosystems and highlights the role of institutions therein.
