Economic evaluation of community programs for the prevention of dental caries in Catalonia, Spain

Abstract: The purpose of the present study was to estimate the cost-effectiveness of three alternative community programs to prevent dental caries among Catalonia's schoolchildren. “…A review of the literature identi® ed eight previous economic evaluations of community water¯uoridation (White et al, 1989). Five of the studies were described as cost-bene® t analyses (Davies, 1973;Dowell, 1976 ;Nelson and Swint, 1976;Carr et al, 1980 ;Doessel, 1985), two were cost-e ectiveness analyses (Horowitz and Heifetz, 1979;Manau et al, 1987), and one undertook both types of analysis (Niessen and Douglass, 1984). In all the so-called cost-bene® t analyses, the only`bene® ts' that were valued were the anticipated savings in dental treatment associated with improved dental health.…”

“…A practical consideration is that the product should be convenient to use and be tolerated by patients. An important aspect of publicly funded school-based programs is the cost relative to the expected benefits (Clark et al• 1985a;Manau et al, 1987;Niessen and Douglass, 1984), and another consideration is safety (Ripa, 1987).…”

“…Of the 39 MPH economic evaluations identified, 5 studies (7 articles) 4 attempted to value the loss of consumer choice [2,16,[31][32][33]36,37] while a further 4 studies mentioned that the introduction of a MHP would result in a loss of consumer choice [3,4,11,34]. One additional study did not mention consumer choice directly, but estimated the loss in terms of people who quit cycling as a consequence of the introduction of mandatory helmet wearing legislation [28] (see Table 2).…”
