“…Allerdings sind die beatmungsbedingten Veränderungen der Nachlast und der Kontraktilität des linken Ventrikels nicht durchgehend als positiv zu betrachten, da eine deutliche Inhomogenität der linksventrikulären Kontraktion mit septaler Hypo-bis Dyskinesie und lateraler Hyperkontraktilität bei raschen Änderungen des Atemwegsmitteldruckes (PEEP-Änderung von 0 auf 16 mbar) auftreten kann (98). Die Inhomogenität der Kontraktilität ist sicher nicht allein durch eine anatomisch bedingte, regional unterschiedliche Druckübertragung auf das Herz verursacht, sondern widerspiegelt, neben der Nachlasterhöhung des rechten Ventrikels mit Störung der septalen Kinetik (23, 31), ventrikeleigene Anpassungsmechanismen (71), Auswirkungen der Blutvolumenverschiebung und Folgen der Größenänderung der Herzhöhlen (18, 97) auch die Abnahme der begrenzenden Funktion des Perikards (42,98) Die ausgeprägtere respiratorische Insuffizienz kann häufig nur durch die Anwendung einer maschinellen Beatmung überlebt werden. Sowohl in experimentellen als auch in klinischen Untersuchungen ist jedoch gezeigt worden, dass die künstliche Beatmung durch hohe Atemzugvolumina (Volutrauma), hohe Atemwegsspitzendrucke (Barotrauma) und durch fehlenden oder zu niedrigen PEEP zu mechanischen und nachfolgend inflammatorischen Schädigungen der Lunge führt (16,19,20,72,87,107,111,112).…”
